In compliance with the requirements of the Australian Nursing and Midwifery Accreditation
Council (ANMAC) and by the Department of Immigration and Citizenship (DIAC), I am writing
this professional reference letter for Ms. Paat. She was employed as a Fulltime Registered Nurse
under my supervision in the Medical Ward from April 10, 2013 to July 7, 2013 rendering 40
working hours a week service in the hospital.

Southern Luzon Hospital and Medical Center, known today as The Medical City, is one of the
prestigious private, tertiary hospitals accredited by the Department of Health (DOH) in Sta. Rosa
Laguna, Philippines. It caters and provides quality care and services to patients that include
Radiology, Pathology, Internal Medicine, Obstetrics-Gynecology, Orthopedics, Dental,
Anesthesiology, General Surgery, Oncology, Pediatrics, Urology, Hematology, Nephrology,
Thoracic, Trauma and Injury and many more.

In relation to the ANZSCO Code 254418- Medical Nurse, she provides direct patient care to all
medical, including surgical patients of all ages. Her qualifications for such role include
completion of a Bachelor’s Degree in Nursing, registration in the Professional Regulation
Commission (PRC). Her competencies for such specialty area were achieved through undergoing
in-house competency assessment annually and continuing professional development courses
outside the hospital.

As a Medical Ward Nurse, she performed the following specific tasks:

 Provides age-appropriate nursing care and treatment to patient within the scope of
nursing practice and based on the institution’s policies and procedures.
 Communicates care and activities to multidisciplinary team members (e.g. laboratory,
blood bank, radiology department).
 Acts as liaison between the patient, significant others and medical team members.
 Assesses, plans and implements interventions for the well-being of individual patients.

 Prioritizes activities based on the individual care needs of patients.


 Provides skilled assistance and intervention in critical and emergency situations.
 Documents accurately all activities in the unit and maintains a clean, safe and
comfortable environment for the patients.
 Provides spiritual and post-mortem care.
 Assists patient in moving, maintaining proper alignment of patient’s body and utilizing
good body mechanics;
 Takes and records vital signs;
 Observes medical sepsis;
 Observes signs and symptoms, institutes remedial action when appropriate and records
these in her chart;
 Reports observations to the physicians;
 Observes and responds to patients’ emotional spiritual needs;
 Interprets to the patient and his family their roles in promoting successful therapy and
rehabilitation;
 Prepared patients for, and assists physicians with, diagnostic and therapeutic procedures;
 Carries out doctor’s legal orders;
 Assists physicians when examining patients;
 Performs irrigations, catheterizations and gives enemas;
 Administers and charts medications and patients’ reactions to medications;
 Instructs patients and families;
 Sees to it that equipment receives proper care and maintenance;
 Discharge patients;
 Receives and endorses ward equipment and supplies;
 Interprets hospital policies and procedures;
 Gives health teachings;
 Maintains good relationships with other hospital personnel;
 Charts accurately medications and treatments given;
 Attends meetings and conferences; and
 Performs other duties as delegated by superiors.

Moreover, she has strength in dealing with work, patients, and colleagues. She is very skilled,
responsible, efficient, enthusiastic, and kindhearted individual. As a Nurse, she constantly
provided safe, empathic and quality nursing care and is always eager to go a step further than
expected. She’s an invaluable part of a team and always kept a professional relationship with
colleagues and other members of the team. She had an admirable attendance record and always

punctual. She also thrives to upgrade practice and develop professionally through constant
attendance in seminars and trainings in and out of the hospital.

To put it briefly, I would recommend Ms. Paat. I believe she will be an efficient addition to any
