3. That she solely provides for the physical, emotional and financial needs of her
children;

4. That she has never been married or cohabited any one and has remained single
since then up to the present;

5. That we are executing this affidavit to attest to the truth of the foregoing
statements and for whatever legally purpose it may serve;
