JOINT AFFIDAVIT OF TWO DISINTERESTED PERSONS

We, MALVIN RONCALES and JAG VICARIO, both of legal age,

Filipinos and residents of Zone 6, Pambujan, Northern Samar, after having
been duly sworn to in accordance with law, do hereby depose and say:

1. That we personally know FORTUNATO EQUIZA and MARY JOY

EQUIZA since they are neighbors and close friends of ours;

2. That we are fully cognizant of the fact that FORTUNATO EQUIZA

and MARY JOY EQUIZA have been living together as married
couple. They were married on January 13, 2006 at Cawayan, Northern
Samar;

3. The parent’s name of FORTUNATO EQUIZA in the marriage

certificate recorded on the Local Civil Registrar and on the National
Statistic Office are Dianne Espano and Richard Balleta;

4. That the correct parent’s name of FORTUNATO EQUIZA that should

appear in the record Local Civil Registrar and in the National
Statistics Office JODITH ABAYON AND NELSON BALLETA.

5. That parent’s name of MARY JOY CASTILLO in the marriage

certificate recorded in the Local Civil Registrar and in the National
Statistics Office are Jodith Abayon and Nelson Castillo;

6. That the correct parent’s name of MARY JOY CASTILLO that

should appear in the record in the Local Civil Registrar and in the
National Statistics Office GRACE MANANGUITE AND JASON
CASTILLO;

7. Fortunato Equiza and Mary Joy Equiza are processing the application
for the correction in the entry of the Marriage Certificate of Fortunato
Equiza and Mary Joy Equiza at the Office of Civil registrar;

8. We attest to the truth and veracity of the foregoing facts and

circumstances. That we execute this affidavit to support the
application of Fortunato Equiza and Mary Joy Equiza for the
Correction in their Marriage Certificate at the Office of the Civil
Registrar and for whatever legal purposes this may best serve.
