It was great to speak with you today for your Information Session. Enclosed you will find the
Information Session Form. On the first page of the Information Session form, please initial next
to A-J, mark which resolution option you choose, initial next to "I have been advised....", and on
the second page, sign and date and place "NR" for Not Responsible next to 6.C.1. Cheating, and
place "R" for Responsible next to 6.G.1. Plagiarism. Once you have done so, please scan and
email that to lbourzac@fiu.edu.
As discussed, you have chosen Summary Resolution as your resolution option and we agreed
you are Responsible for 6.g.1. Plagiarism. If you have any questions, please feel free to reach out

Days ( ) to consider your options and to notify this office in writing of your option choice.
Failure to do so by the above date will result in SCAI designating the hearing format and proceeding with
scheduling the hearing for you.
I have been advised and understand that my failure to appear at a scheduled hearing will result in the hearing
being held in my absence.

The charges against me/my organization have been explained to me, and I understand them. I have had the
opportunity to review the information SCAI/Housing and Residential Life (HRL) currently possesses and which
may be presented at the hearing (i.e., University police reports, housing incident reports, emails, texts, photos,
videos). I understand that SCAI/HRL may receive additional information which may be used in the hearing however
SCAI will provide me with the list of evidence no later than three (3) Business Days prior to the hearing. I have also
been advised of my rights under the Student Conduct and Honor Code and have selected my resolution option. I
further understand that this Information Session Form is only a summary of the Student Conduct and Honor Code,
