have had an opportunity to ask questions, and that you understand these rights.)
 
NM (a) Reasonable, written notice of the Code charge(s) and the allegations upon which the charge(s) is based.
NM (b) A fair and impartial hearing.
NM (c) The opportunity to be accompanied by an Advisor of your choice and at your expense at any time during
the Code process. (Our office can provide you with a list of advisors who are familiar with the Code
process and who can give assistance in answering your questions about the process and/or accompany you
to your hearing. If you are interested in this list, please notify our staff.)
NM (d) The opportunity to review all relevant information or evidence to be used in the hearing prior to the
hearing.
NM (e) The ability to participate in the Student Conduct hearing either physically or by contemporaneous
alternative means (e.g. Skype, phone).
NM (f) The opportunity to present relevant Witnesses and information at the hearing.
NM (g) The opportunity to question Witnesses in accordance with the Hearing Procedures.

Invoking the right against self-incrimination will not be considered as a negative factor in the decision of
the Hearing Officer or Hearing Body.

of the hearing.
NM (j) Appeal the decision via the process established by the University.

I have been advised and understand that my failure to appear at a scheduled hearing will result in the hearing
being held in my absence.

The charges against me/my organization have been explained to me, and I understand them. I have had the
opportunity to review the information SCAI/Housing and Residential Life (HRL) currently possesses and which
may be presented at the hearing (i.e., University police reports, housing incident reports, emails, texts, photos,
videos). I understand that SCAI/HRL may receive additional information which may be used in the hearing however
SCAI will provide me with the list of evidence no later than three (3) Business Days prior to the hearing. I have also
been advised of my rights under the Student Conduct and Honor Code and have selected my resolution option. I
further understand that this Information Session Form is only a summary of the Student Conduct and Honor Code,
