AFFIDAVIT OF UNDERTKING

We, (Name of Student), (Year, course and section) and (Name of Parent/Guardian/Co-maker),
both of legal ages, and Filipinos, and residents of (Address), after having been duly sworn according
to law, hereby depose and state that:
1.) The connectivity kit the student has received and accepted last ___________ bearing serial
number __________________ is a property of CTU _______ Campus;

2.) This connectivity kit shall be primarily used for the CTU- Flexible Learning System (CTU-
LMS) with the main objective to provide learners with the most flexibility on the learning
content, schedules, access, innovative assessment and making use of digital tools.

3.) In case the kit malfunctions, the student shall immediately coordinate with the adviser for
possible repair/replacement through the Campus MIS Office.

4.) The student through its program chairman or class adviser shall surrender the connectivity
kit at the end of every semester during the specific schedule announced for this purpose.

5.) Prior to transfer to another school or after graduation, the student shall immediately
surrender the said connectivity kit during the specific schedule announced for this purpose.

6.) In case of loss or irreparable damage to the connectivity kit, the

student/parent/guardian/co-maker shall immediately approach and notify the adviser for
reporting to the Campus MIS Office.

7.) The student/parent/guardian/co-maker hereby agree and assume any and all risk of loss,
damage and liability whether intentional or unintentional that may occur to the said
connectivity kit.

8.) Further, the student/parent/guardian/co-maker will be jointly and severally liable to take
charge in the payment/replacement of the connectivity kit provided in the form of a loan.

9.) We further undertake and understand the terms and responsibility set forth and hereby
waives any rights freely and voluntarily to the fullest ever allowed by law.
