AFFIDAVIT
WE, WILFREDO S. MARASIGAN, DENNIS MARSOL S. MARASIGAN
and MARIA CRISTETA S. MARASIGAN all of legal age, Filipinos, and residents of
Cuta, Batangas City and San Pascual, Batangas, after having been duly sworn to in
accordance with law hereby depose and state:

That we are the children of the late JOSE M. MARASIGAN who died on
December 7, 2007, copy of the Certificate of Death is hereto attached to form part of this
Affidavit;

That our father when he was still living is a regular member of the Social
Security System and as such, he is entitled to all the benefits due him along with his
dependents;

That upon his retirement, he is regularly receiving his monthly pension until the
time of his death;

That as a requirement in availing of the continuous pension, we have to include at

least three (3) Certificates of Live Birth attesting thereto the DATE OF MARRIAGE of
our parents;

That in my (Wilfredo Marasigan) Certificate of Live Birth, my date of birth is

May 4, 1970, while the date of marriage of our parents is January 2, 1948;

That in my (Dennis Marasigan) Certificate of Live Birth, my date of birth is

September 24, 1974, while the date of marriage of our parents appearing therein is June
30, 1948;

That in my (Maria Cristeta S. Marasigan) Certificate of Live Birth, my date of

birth is June 24, 1977, while the date of marriage of our parents appearing therein
January 1, 1947;

That unfortunately, our Certificates of Live Birth bear different DATE OF

MARRIAGE of our parents due to inadvertence as our father is the only breadwinner of
our family during that time and he was the one informing the datum to go along with his
job at Caltex Refinery;

That this statement was made to inform proper authorities/agency of the

erroneous entry particularly the DATE OF MARRIAGE of our parents in our respective
Certificates of Live Birth was due to reason above-stated and not to evade criminal and or
civil liability but for the purpose of securing or for the transfer of the monthly pension to
our mother, CRISTETA S. MARASIGAN.

That we are executing this affidavit to prove the foregoing matters.
