AFFIDAVIT/CONSENT

WE, CELSO ABUSTAN y Esparago and MARIA DOLORES SANTOS y

Serrano, both of legal age, married, Filipinos and residents of Brgy. Libjo,
Batangas City, after having been duly sworn to in accordance with law hereby
depose and state:

That we are the parents ot JUAN JEROME MIGUEL SANTOS, 17 years old,
single and resident of Barangay. Libjo, Batangas City;

That we are allowing our child, JUAN JEROME MIGUEL, to be employed by

CHOWKING RESTAURANT BATANGAS, and to apply for his Health Card which is
a requirement for his employment;

That we are executing this affidavit to prove the foregoing matters and to
give our consent to the employment and application of our child, JUAN JEROME
MIGUEL SANTOS.
