AFFIDAVIT OF SUPPORT

I, WILLIAM P. LAZA, of legal age, Filipino, married and a resident

of 117 Vergara Street, Batangas City after having been duly sworn to in
accordance with law, hereby deposes and states:

That I am the maternal uncle of minor, SAM CHRISTIAN L.

TRASPORTES who was born on October 18, 2001 and is now living with
me;

That SAM CHRISTIAN TRASPORTES is the legitimate son of

Spouses Samuel Trasportes and Mauricia Laza;

That the said spouses separated in fact and the custody and care of
minor Sam Christian was left with her mother, Mauricia Laza;

That during their separation in fact, Mauricia Laza suffered from

breast cancer and died on September 9, 2008;

That I was the one who shouldered the medical and burial expenses of
my aunt and the custody of the said minor was left with me;

That I am now the one

That my daughter will travel to New Orleans, Louisiana, USA to

finally settle and live with me;

That my child will be leaving the Philippines sometime the middle of

September, 2008 with my husband VENANCIO HEDELITO M. TAGLE;

That I am giving my full consent to the travel of my daughter to New

Orleans, Louisiana, USA who will accompanied by my husband;

That I am executing this affidavit to attest to the truthfulness of the

foregoing facts and circumstances and for whatever legal purpose it may
serve.
