This is an open book examination. It is not invigilated, and you are free to use any
learning resources to which you have access, eg your course notes, or the HSE
website, etc.

By submitting your completed examination for marking, you are declaring it is entirely your
own work. Knowingly claiming work to be your own when it is someone else’s work is
malpractice, which carries severe penalties. This means that you must not collaborate with
or copy work from others. Neither should you ‘cut and paste’ blocks of text from the Internet
or other sources.

The examination begins with a realistic scenario to set the scene. You will then need to
complete a series of tasks based on this scenario. Each task will consist of one or more
questions.

Your responses to most of these tasks should wholly, or partly, draw on relevant information
from the scenario. The task will clearly state the extent to which this is required.

The marks available are shown in brackets to the right of each question, or part of each
question. This will help guide you to the amount of information required in your response. In
general, one mark is given for each correct technical point that is clearly demonstrated.
Avoid writing too little as this will make it difficult for the Examiner to award marks. Single
word answers or lists are unlikely to gain marks as this would not normally be enough to
show understanding or a connection with the scenario.

The examination will take you approximately 4 hours to complete. This includes reading,
researching, and completing the tasks. You are not expected to write more than 3 000
words in total.
Try to distribute your time and word count proportionately across all tasks.

SCENARIO

In a recently opened supermarket, during the time of working hours, a 24-year-old girl who was
employed in that supermarket and slipped on water leaking from an ice-storage and freezing
machine. Due to slips at workplace, she was admitted to hospital and found leg fractured. She was
not able to attend her duties for next 1 month.
Supermarket floor was most affected from slips hazards due to new & slippery floor tiles.

There are 15 staff who works at various zones inside the supermarket. There are 3 managerial staff
includes 1 Cash handling manager, 1 customer service manager and 1 supermarket supervisor.
The supermarket supervisor who takes care of staff attendances & monitoring, Health and Safety
related issues and safe execution of all activities in the Supermarket.

The supermarket was short staffed on the day of accident and the supermarket supervisor was
working on the checkout counter instead of monitoring workplace safety. Although the Supermarket
policy was to mop up the spillages. it was common practice to leave spillages at busy times and
cover them with a sheet of cardboard, which itself can create a tripping hazard. At busy times it was
usual to give greater priority to serving customers than to cleaning spillages. The various suppliers,
vendors also used to visit into the supermarket to display their products in the displaying areas for
marketing their products.

The ice-storage & freezing machine had been leaking for several days and no attempts had been
made to cure the leak. No one had sole responsibility to co-ordinate the repair of faulty equipment
and a lack of communication between the managers and staff left the equipment leaking over a long
period of time. The supermarket supervisor, who had a good awareness of slips and trips risks and
an efficient system for identifying spillages.

Investigations showed that floor of supermarket quickly used to become wet on rainy days with
water being walked in on shoes through the lobby of supermarket. The entrance matting systems in
place were not large enough to cope with the amount of water transferred onto the mats. The
enforcement authority prosecuted the Supermarket management. And on successful conviction, the
enforcement authority imposed a fine on supermarket management and assisted the girl with the
compensation.
After this incident, the Supermarket management hired a dedicated Health and Safety Officer to
maintain all Health and Safety standard in future. The Health and Safety officer believed that the
accident was completely avoidable, as the Supervisor had failed to maintain a safe system of work
or to carry out a suitable and sufficient assessment of the risks associated with slipping within the
supermarket. Simply cleaning up this spillage, and drying the small area of floor with a paper towel
would have been far better.

This newly appointed Health and Safety Officer, formed a Health and Safety committee to
implement and maintain the health and safety in Supermarket. Health and Safety Officer
recognized that success would rely heavily on the involvement and commitment of the whole of the
workforce. He informed all the staff of supermarket to identify the risk of the slips & trips in their
working zone and set a target of reducing slips & trips within a year. Staff working groups were
asked to hold their own meetings and to look for problem areas, opportunities for improvement and
suggest possible solutions.

It was found sometimes that supermarket staff cleaning the wet area on their own without the need
to report to Supermarket supervisor or Health and Safety Officer.
