You are hereby required to appear before the undersigned at the

OFFICE OF THE CITY PROSECUTOR, 3RD Floor, Hall of Justice, City Hall
Compound, City of Mandaluyong on 15th day and 22th day of April 2019
at 10:00 AM., then and there to testify under oath or answer clarificatory
question in the preliminary investigation to be conducted in the above-
entitled case.

Pursuant to paragraph 3 (c), Rule 112 of the 1985 Rules on Criminal

Procedure, you are hereby required to submit within ten (10) days from
receipt of this subpoena and enclosed affidavit/s and other supporting
documents. The said counter affidavit/affidavit shall be sworn to before any
prosecutor, state prosecutor or government official authorized to administer
oath who must certify that he/she is satisfied that the voluntarily executed
and understood their affidavits and to appear for preliminary investigation
before the undersigned on the above date and time and then and there
testify under oath at answer clarificatory question if necessary.
