REGION MEMORANDUM
HRDD No. 35, s. 2020

SUBMISSION OF REQUIREMENTS FOR LEARNING AND DELIVERY MODALITIES

(LDM) COURSE 1 FOR DIVISION AND SCHOOL LEADERS
TO: Schools Division Superintendents

1. This has reference to the Regionwide Rollout of the Learning Delivery

Modalities (LDM) 1 Course for Division and School Leaders as per Region
Memorandum HRDD No. 28, s. 2020.

2. As the LDM 1 Course culminates, participants are required to submit a

portfolio of outputs from Modules 1 to 7 to the Human Resource Development
Division (HRDD), DepEd SOCCSKSARGEN Regional Office on or before September
11, 2020.

3. The submission shall be done electronically through this link:

http://bit.ly/LDM1Portfolios. However, all concerned individuals shall keep a
hard copy of the documents to be submitted when situation already permits.

4. The electronic portfolios shall have the cover page using the attached
template. Please see Enclosure No. 1.

5. In the preparation and submission of requirements, please be guided by

Region Memorandum HRDD No. 28, s. 2020, paragraphs 11, 11.1, 11.2 and 11.3, to
wit:
11. For the Completion and Award of Certificate of Participation, as indicated in the
implementation guidelines, the coaches will have the primary responsibility for the evaluation
of the participants in the form of submission of requirements and/or assessment. The
Assessment shall be on a per Module basis.

11.1 For Division and District Supervisors, the individual portfolio shall be collected
by LAC leaders, submitted to the Division EPSs in-charge of LAC who shall endorse to the
RFTATs for evaluation.

11.2 For School Heads, the individual portfolio shall be collected by LAC
leaders to be submitted to CID Chiefs and Division EPSs in-charge of LAC for
evaluation.
11.3 The results of evaluation shall be forwarded to Mervie Y. Seblos, HRDD
OIC-CES and NEAP-RXII Focal Person for validation. The certificate of participation will
be given to the participants that have successfully completed the course and/or passed
the assessment. The certificate will indicate professional development credit units that

will be recorded in the participants’ professional development account as part of the
Professional Development Information System (PDIS), as may be applicable.

5A. For Division and District Supervisors, please refer to Enclosure No. 2
for the list of requirements to be included in the individual portfolios.

5B. For School Heads, please refer to Enclosure No. 3 for the list of
requirements to be included in the individual portfolios.

6. It is reiterated that the individual portfolios of the division and district

supervisors shall be evaluated by the RFTATs of the Regional Office while the
individual portfolios of the School Heads shall be evaluated by the CID Chiefs and
Division LAC Coordinators. Thus, only the individual portfolios of Division and
District Supervisors shall be sent for evaluation to HRDD through the link provided
in paragraph 3 of this memorandum. On the other hand, the individual portfolios of
the school heads shall be evaluated by the CID Chiefs and Division LAC Coordinators
and only the evaluation results shall be sent thru email to:
mervie.seblos@deped.gov.ph for validation purposes. The individual portfolios of the
school heads shall be retained at the CID offices for reference in the Part II:
Practicum-LDM Implementation.
