accordance with law, do hereby depose and say:

1. That, I am an owner and proprietress of a vegetable stall located at

Daang Amaya 1, Tanza, Cavite;

2. That, I derive income in the above-mentioned businesses;

3. That, I am earning a monthly gross income in the amount of ONE

HUNDRED EIGHTY THOUSAND PESOS (Php 180,000.00) and with net
income of SIXTY FIVE THOUSAND PESOS (Php 65,000.00) broken down
as follows:

Rent = 6,000.00

support my application for business/salary loan to SMARTMAB LENDING
CORP.
