accordance with law, do hereby depose and state:

1. That I am the proprietor of ________________ with business address

at _____________ which is involve in the buying and selling of
Palay/Rice;

2. That last year 2019 our business has tremendously slow down, the
transaction were few and far between so much so that our income has
dropped to its lowest level;

3. That our business has been affected negatively by the influx of

imported rice and the difficulty of selling local rice at higher price;

4. That this affidavit is executed in good faith and intended for no other
purpose other than to express the truth;

5. That I am executing this affidavit to attest to the truth and to justify

the production of business transaction and income of the above
mentioned enterprise.
