Slip & trip while guiding of


Hydraulic Rig movement
Incident Details :
At pipe rack 105, G-5 socket Piling sub-contractor HSE officer who was monitoring reversing
of Rig-05 (HR-180) machine. During this process, he stepped on the metal shuttering plank
by mistake. Plank was placed on pipe rack column pocket as a protection. During this his left
leg slipped and about to fall on ground, which he controlled later. This has occurred due to
the displacement of metal shuttering planks. No injury resulted.

Probable cause :
•Not anticipated the risk of stepping on the column socket pit metal cover
• Lapse, mistake & omission
• Column socket pit cover was not in correct position & secured.

Recommendations :
• Shuttering plate covers to be fully secured.
• Barricading of these pockets to be ensured everywhere.
• Risk shall be communicated to all during tool box meeting.
