Ukraine Family Visit Visa Checklist

Documents for all applicants:

1. Passports: Original Passport with a validity of minimum 6 months from the intended date of
departure with at least two blank pages.
 Previous passports
 Copies of previously acquired visas

2. Visa Application Forms: Fill online visa form at the website https://visawebapp.boca.gov.tw and take
the printout of filled online form, should be signed by the Applicant (we will help)
3. Photograph: 2 recent Color photograph in 35 mm x 45 mm in size with white background.
4. Ticket Itinerary: A reserved itinerary for your airline ticket as per the schedule of the trip.
5. Health Insurance: To cover the possible expenses related to repatriation, for health reasons, urgent
medical assistance and/or urgent hospitalization – For single / double entry visa – covering the entire
period of stay in Ukraine.

Documents For The Main Applicant:

1. Cover Letter: A detailed cover letter depicting your socio-economic profile, professional and social
status, capacity to support yourself and your family members during your visit. (VISAThing will help
you, if needed)
2. Financial Documents: Documents proving your capacity to financially support yourself during your
stay in Ukraine. In your case, please include the documents as listed below
 Personal Bank Statements (for last six months)
 FDR Certificates/National Deposit Scheme Certificate (if any)
 Copy of ITR (Income Tax Return) for last three years (if any)
 Notarized Asset Declaration with summary of wealth evaluation statement

3. Supporting Documents: Supporting documents listed below shall be attached with the file for added
support to your visa application.
 Marriage Certificate
 A VISA requesting letter from your Wife (we will help)
 Family photographs
Documents For Your Wife:
 Bangladesi visa copy/ entry permit copy

Documents For Your Mother:

 Personal Bank Statements (for last six months)
 FDR Certificates/National Deposit Scheme Certificate (if any)
 Copy of ITR (Income Tax Return) for last three years (if any)
 Notarized Asset Declaration with summary of wealth evaluation statement

Documents For The Children:

 Birth Certificate copy

Documents of the relative living in Ukraine:

 Invitation Letter: Invitation Letter from host giving his contact details stating with
the purpose of travel, duration of stay and showing responsibility for your
accommodation and maintenance costs during your stay for visiting Friend/Family
category
 Ukraine Passport copy/National ID copy
 Contact details
 Residence utility bill copy
 Any other documents that proves his identification/residency status
