HOW TO FILL UP THE DATA PRIVACY AND ANTI-CYBER BULLYING

AGREEMENT

1. Download & print a copy of the Data Privacy and Anti-Cyberbullying Agreement
(available at your e-mail registered with FEU-NRMF).
2. Carefully read and understand the content of the Data Privacy and Anti-
Cyberbullying Agreement.
3. Fill in the blanks with the needed information. (Please refer to the illustration picture
below)
1. Day you signed the agreement (example – 12th)
2. Month you signed the agreement (example – October)
3. Student complete name – (example – JUAN DELA CRUZ)
4. Address of the student – (example – St. Michaels St. Republic Ave. Holy
Spirit Quezon City)
5. Complete name and signature of the student/parent/guardian (Note: If the
student is below 18 years old the parent or authorized guardian will be the one to
sign on this portion. E-signature is accepted also)
6. Signature over printed name of witness 1 – (Note- witness 1 is any person
whom you know and at least 18 years of age)
7. Signature over printed name of witness 2 – (Note- witness 2 is any person
whom you know and at least 18 years of age)
4. Once the document is completed, scan it and put a file name. The file name shall
be in this format – (student number plus complete name of the student) example -
20190065601JUANDELACRUZ)
5. The student is required to keep a PDF copy of the document submitted to DPO.
6. Submit the scanned document to dpo@feu-nrmf.edu.ph
7. The deadline of submission is on October 30, 2020.
