LETTER OF REQUEST

Subject: (Requirements for School Applications)

My child/ward, Josue Marie N. Venida is a/an incoming Grade 12 STEM student enrolled in City of Mandaluyong

Science High School and is about to apply to other schools/universities for college, so I would need certain

documents for the requirements for my child/ward’s school applications. Below is the list of documents I need

from your end:

· Transcript of Records (Grade 7 – Grade 11)

· Certificate of Batch Ranking

· Certificated of Strand Ranking

· Certificate of Good Moral

· Leadership Certificate (Pangalawang Patnugot in Teknogham)

· Form 138 or 137 / Grade 11 Report Card

· Three Recommendation Forms (one from the principal, one from the guidance counselor or class

adviser, and one from any of teachers

I would like to receive a confirmation regarding this at the earliest. Also, I am attaching my ID with a signature

as a proof of guarantee that I’m the parent/legal guardian. More so, I will be responsible in receiving the

aforementioned documents.
