MALNUTRITION

*171. SHRI DEEPAK BAIJ:

Will the Minister of WOMEN AND CHILD DEVELOPMENT be pleased to state:

(a) the details of malnutrition among children aged 0-5 years in the country and the
programmes put in place to combat malnutrition;
(b) the details of bottlenecks in accessing schemes aimed to combating
malnutrition;
(c) the details of increase in honorarium of employees like Anganwadi Sahayika
and the cook in charge of preparation of mid-day meal as part of schemes
launched to combat malnutrition, during the last five years along with the details
of honorarium being paid to the said employees at present; and
(d) whether in view of the current situation of malnutrition among children, the
Ministry propose to make serious efforts to ensure that the benefits of the said
schemes reach the affected people so as to eradicate malnutrition?

171 for answer on 29.11.2019 by SHRI DEEPAK BAIJ regarding Malnutrition

(a) to (d) As per National Family Health Surveys (NFHS-4) conducted by the
Ministry of Health and Family Welfare, 35.7% children under 5 years of age are
underweight, 38.4% are stunted and 58.5% children are anaemic indicating a
reduction from the previous NFHS – 3 conducted in 2005-06, which reported 42.5%
children under 5 years of age as underweight, 48% stunted and 69.5% as anaemic.
As per the report of Comprehensive National Nutrition Survey (CNNS) (2016-18),
prevalence of stunting, wasting and underweight among children is 34.7%, 17%
and 33.4% respectively, which is a reduction from the levels reported by NFHS -4.

Malnutrition is a complex and multi-dimensional issue, affected by a number of generic


factors including poverty, inadequate food consumption due to access and availability,
inequitable food distribution, improper maternal infant and child feeding and care
practices, inequity and gender imbalances, poor sanitary and environmental
conditions; and restricted access to quality health, education and social care services.

The Government has accorded high priority to the issue of malnutrition and is making
serious efforts to eradicate malnutrition. Several schemes with direct as well as
indirect interventions are being implemented across the country. Schemes like
Anganwadi Services, Scheme for Adolescent Girls and Pradhan Mantri Matru
Vandana Yojna (PMMVY) under the Umbrella Integrated Child Development Services
(ICDS) Scheme as direct targeted interventions are being implemented to address the
problem of malnutrition among women and children in the country.

Anganwadi Services, aims to improve the nutritional and health status of children in
the age-group 0-6 years, pregnant women & lactating mothers and reduce the
incidence of mortality, morbidity and malnutrition. Under the Scheme, a package of six
services is provided through a network of Anganwadi Centers, which include
supplementary nutrition, pre-school non-formal education, nutrition & health
education, immunization, health check-up and referral services. National Food
Security Act, promulgated in 2013, has mandated the nutritional entitlement for the
beneficiaries of Anganwadi Services as legal entitlement. The scheme is universal
and open to all the eligible beneficiaries. A total number of 1.60 crore women and 6.75
crore children are benefitted under the Anganwadi Services as on 30.06.2019
(Annexure-I)

Scheme for Adolescent Girls is implemented for out-of-school adolescent girls in the
age group of 11-14 years for self-development and empowerment of adolescent girls;
improvement of nutritional and health status; to promote awareness about health,
hygiene & nutrition; support to out of school Adolescent Girls for successful transition
back to formal schooling or bridge learning/skill training; upgrade their home-based
skills and life skills. A total number of 4.76 lakh Adolescent Girlsare benefitted under
the SAG scheme as on 14.11.2019 (Annexure-II)

The objective of Pradhan Mantri Matru Vandana Yojana (PMMVY) is to provide


cash incentive to improve health seeking behaviour amongst the Pregnant Women &
Lactating Mothers (PW&LM) and to compensate partially the wage loss in terms of
cash incentive so that the woman can take adequate rest before and after delivery of
the first child. A total number of 1.11 crore beneficiaries have benefitted under
PMMVY and Rs. 4,501.50 Crore have been dispersed to the beneficiaries as on
27.11.2019 (Annexure-III)
Government has set up POSHAN Abhiyaan on 18.12.2017 for a three year time
frame commencing from 2017-18 with an overall budget of Rs. 9046 Crore. To ensure
a holistic approach, all 37 States/UTs and districts have been covered. The goals of
POSHAN Abhiyaan are to achieve improvement in nutritional status of Children from
0-6 years, Adolescent Girls, Pregnant Women and Lactating Mothers in a time bound
manner during the three years with fixed targets as under:

The major activities undertaken under the POSHAN Abhiyan are Community
Mobilization, creating Awareness, Advocacy leading to Jan Andolan- to educate the
people on nutritional aspects. Community events are conducted and for creating
awareness, POSHAN Pakhwada and POSHAN Maah are observed. The Poshan
Maah was organised in September 2019 and received a massive response from the
participants which is also evident from more than 3.4 million impressions generated
on Twitter and 8,60,683 impressions generated on Facebook during the month. In
addition, an awareness campaign on Doordarshan and Digital Cinema was also
carried out.Community radio was also involved to increase the reach of messaging of
nutrition during the month.

Under the National Health Mission (NHM), a flagship programme of Ministry of


Health and Family Welfare, programmes to address malnutrition inter alia include
promotion of appropriate Infant and Young Child Feeding (IYCF) practices, Anaemia
Mukt Bharat, Iron and Folic Acid (IFA) supplementation, de-worming, promotion of
iodized salt, Vitamin-A supplementation, Mission Indradhanush to ensure high
coverage of vaccination in children, conducting intensified diarrhoea control fortnights
to control childhood diarrhoea, management of sick severely malnourished children at
Nutrition Rehabilitation Centres, monthly Village Health and Nutrition Days, Home
Based New Born Care (HBNC) and Home Based Young Child Care (HBYC)
programmes, Rashtriya Bal Swasthya Karyakram, etc.

The Ministry has taken several initiatives to address bottlenecks in accessing the
benefits of the schemes through convergence with various schemes/programmes.
Further, the Ministry has taken initiatives for incentivizing States/ Union Territories for
achieving targeted goals; Information and Communication Technology enabled
Integrated Child Development Services-Common Application Software (ICDS-CAS) for
strengthening service delivery and interventions; Behaviour Change Communication,
Awareness Advocacy and Information Education Communication; Jan Andolan
through Community mobilization to educate people on nutritional aspects and Training
& Capacity Building of frontline functionaries etc.
Anganwadi Services envisages the Anganwadi Workers & Helpers as honorary
workers from local community who come forward to render their services, on part time
basis, in the area of child care and development. Being honorary workers, they are
paid monthly honorarium as decided by Government from time to time. The monthly
honorarium paid to Anganwadi Workers (AWWs) and Anganwadi Helpers (AWHs) was
revised w.e.f. 1st October, 2018 as per details given below:

In addition to the above, States/UTs also pay additional honorarium to the


AWWs/AWHs from their own resources as per details given in Annexure IV.

Cook-Cum-Helper (CCH) under the Mid-Day Meal Scheme (MDMS) are honorary
workers who come forward to render social services. Cook-Cum-Helper is paid
Rs.1000/- per month for 10 months in a year with effect from 01.12.2009 as
honorarium. States/UTs also supplement the honorarium by providing additional funds
from their own resources. The details of the honorarium being paid to the CCHs during
2018-19 working under MDMS in all States and Union Territories are given at
Annexure V.
