SUBJECT: Suspension Letter

TO: Mr. Daryl Jaye Langaman

Head Nurse – Nephrolife Dialysis Center

This letter is to serve as a Suspension Letter to you due to job abandonment as the Head Nurse
of Nephrolife Dialysis Center Co. secondary to the failure to inform your Clinical manager ahead
of time that you can’t come to work last Tuesday (9/22) and Wednesday (9/23). Prior to these
occasions, you and the whole team of the center attended a meeting with Dra Mara Tugade
about absences without prior notice. Furthermore, last Tuesday you had a conversation with
Dra Mara over the phone confirming that you will come to work the next day. Then on
Wednesday there was a clear instruction to you to go to work because there are a lot of
problems going on in the center and you have to help your manager. In the end, you didn’t
show to work. Being one of the senior nurses in your center and the head nurse, the company
is expecting you to be a role model to your junior staff.

Your suspension will run effectively on 9/28 until 10/10. Prior to your return, you will secure
your new center of assignment and schedule through Doc Robert and Dra Mara on 10/12/2020
via meeting.
