The patient reported an 8/10 pain level from a surgical incision and episiotomy sutures. The nursing care plan was to provide interventions over a 7 hour shift to reduce the patient's pain level. Interventions included positioning, breathing and pelvic floor exercises, warm baths, and pain medication. The goal was to lower the patient's pain level to at least 5/10 with less facial grimacing and discomfort after interventions. Evaluation found the patient's pain level was reduced to a 5/10 with less facial grimacing and discomfort, meeting the goal of the nursing care plan.
