OBJECTIVES OF FIELD WORK

• To have an understanding of the nature of Agency’s work and their functions.

• To understand the pattern of substance abuse and its related problems

• To assess the needs and challenges of the clients registered at the agency

• To improve one’s own knowledge and skills required for the practice of social work

INTRODUCTION:

The Social work trainee was placed at Samaritan Society Of Mizoram to have his fieldwork
with one co-trainee Esther F Vansawmpuii.The trainee started his field work from 5 th
September 2019 till 1st November 2019(26 Days) and individual conference was held with
the faculty supervisor Mr. M.Lallawmzuala to discuss and make plans for the fieldwork. The
main objective was to have an understanding of the nature of Agency’s work and their
functions, to understand the pattern of substance abuse and its related problems, to assess the
needs and challenges of the clients registered at the agency and to improve one’s own
knowledge and skills required for the practice of social work.

ABOUT THE AGENCY:

• Back in the year 1980’s, a group of four friends in Bungkawn, Aizawl started a
philanthropic work by collecting the donations from well wishers and distributing
new dresses bought with this donations to the poor, which later paved the way for the
birth of this voluntary organization.

• But with the passage of time, Firm and Societies registration was done on 1995 with
the hope of increasing this philanthropic work.

• After this registration, the organization started to take projects and became one of the
most well known agencies in the State.

WORK DONE:

1. RAPPORT BUILDING: Rapport is a connection or relationship with someone else.
It can be considered as a state of harmonious understanding with another individual or
group. The trainee tried to have a rapport building with the staffs of the agency but the
rapport between the staffs and the trainee happens naturally. The trainee had that
experience where we ‘hit it off’ or ‘get on well ‘with somebody else without having to
try. The friendship starts with the staff by finding common ground, and being
empathic.

2. NEEDLE/ SYRINGE EXCHANGE: The trainee along with one peer educator and
one outreach worker carry out needle syringe exchange programs. Needle and syringe
programs (NSPs) provide people with sterile needles and syringes to reduce
transmission of HIV and other blood borne viruses from sharing injecting equipment.
Though it is carried out for the clients registered in the society, it also includes
substance abusers who are registered to other agencies. This is mainly done at hotspot
sites and at outreach programs.

3. BEHAVOURAL CHANGE COMMUNICATION: The trainee also done Behavior


change communication (BCC) process which is an interactive process with individual
to develop tailored messages and approaches using a variety of communication
channels to develop positive behaviors and maintain appropriate behaviors.

4. ORAL SUBSTITUTION THERAPY (OST): The agency distribute OST every


Monday to Saturday from 10:00 am to 1:00 pm and the trainee helps the Nurse in
distributing OST by stamping a seal to the record book of each client of the agency
and Secondly the trainee also done unpacking of OST tablets and packing of OST for
those who are as Home Dose.

5. VISITING ART CENTER AT KULIKAWN: The trainee along with Nurse and
Outreach worker visited Kulikawn ART center and here six clients of Samaritan
Society of Mizoram those who are taking ART treatment are transfer to Civil Hospital
ART center and the trainee have time to see the medicine taken by HIV Positive for

the first time, unfortunately the trainee could not meet the staff so, no further
information is taken.

6. OUTREACH: - During the fieldwork on every Thursday the trainee had outreach
along with outreach worker and peer educator. Clients were visited in which syringe
needle exchange, hotspot visits and home visits were done.

7. HOME VISIT: During home visit we had interaction with the client and exchange
syringe and several question was asked such as how is their health condition, how
many times they inject in a day, did they have any sexual partner, Did they shared
syringe with anyone else and condom was also distributed

8. MEETING : -

a. MONTHLY: Monthly review meeting was held once in a month and the
trainee attend the meeting at the Migrant Office along with all the staff and in this
monthly review meeting OST center will give report about every details of the work
they done and also the Migrant staff gives report about every details of the work done
in the past month.

b. WEEKLY : Weekly Review Meeting was held every Friday and here all the peer
educators gives their weekly activity report such as how many Syringe has been
distributed, number of condoms distributed , numbers of one to one interaction, one to
group interaction and how many clients they have , number of taking OST , number
of taking ART , how many client are in rehabilitation center , numbers of clients those
cannot be contacted and after that the Project Manager will give encouraging speech
and make correction in their mistake.
c. OST CLIENT MEETING: Every Saturday OST client meeting was held and the
meeting started from 10:30 am to 1:00 pm and here the Program was started by Bible
reading and prayer and the Project Manager will give lecture on different topics which
are very important for the client such as Differentiation of Drug, withdrawal
symptoms of No.4 and important of OST also it effects and benefits.

d. FAMLY MEETING: - Family meeting was also held and here especially the
families from those who are taking OST as Home Dose are invited and the project
manager and the project director gave speech about the importance of family support
and many things were discuss in this meeting.

9. CONDOM DEMONSTRATION: The trainee was taught how to demonstrate


condom using a dildo by the outreach worker and the trainee also do the
demonstration of condom with the new client using dildo.

10. CASE STUDY : The trainee conducted case study during the fieldwork

PROFILE

FINDINGS

• The trainee found that the client started doing drugs due to peer pressure and his first
drug use is called parvon.
• As time passes, the client started using injecting drugs daily and became an addict in
which his daily dose is of 4 injections per day
• The client started to face discrimination from the society and from his own family as
drug addiction took over his life.
• The client face withdrawal Symptoms and sometimes he want to cease the use of
drugs.
• The client faced legal problems, social problems as well as health problems.
• The client started to take OST from Samaritan agency but unfortunately after 4 weeks
he relapse and started again taking injecting drugs along with pills,and the client

started again taking OST and now live a normal life and the client daily dose is 10mg
per day

CASE 2

PROFILE

• Name : ABC

• Age : 33

• Sex : Male

• Marital Status : Unmarried

• Denomination: Salvation Army

• Qualification : Class V

• Occupation : Cobbler

FINDINGS

• The client is a HIV +ve patient which he receive due to Sex

• He started doing drugs due to his siblings addiction of proxyvon, so his first drug use
is proxyvon.

• The client daily dose is of 3 injections per day

• The client started taking OST from Samaritan agency since 2017 and the client also
registered in shalom agency to take his ART.

• He has mental problems particularly depression in which sometimes he tried to take


his own life.

• The client current CD4 count is 1056.

EVALUATION OF THE AGENCY:

From the very first day of our fieldwork, the very first thing that the trainee found is
that there is a good relationship among the staffs. The punctuality and the discipline are
splendid. With 418 clients registered in the agency, the administration of the agency in
handling each and every client is pretty impressive. They are expert in handling Oral
substitution therapy (OST) and other fields such as OST client meetings, Needle and syringe
exchange, Outreach works etc.; Above all they have a huge understanding for others, as
understanding client’s problems is vital in social work.

On the other hand, the opening of the office is at 10:00 am and a handful of clients are
daily workers, so there is a time problem. During my 26 days placement in the agency, there
are at times losses of sincerity within the staffs when the project manager is not around. The
location of the agency is hard to access as it is a bit remote.

The agency doesn’t have support from a handful of the community where signature
campaigns are held to stop the agency, as they didn’t want gathering of drugs abusers near
them. Local NGO’S sometimes stops the syringe exchange the agency carried out. The
agency needs interventions from experts and celebrities to raise awareness among the people
that they are the ones who save drugs addict’s lives and the one that can take down our state
from the highest HIV prevalence state in the country.

EVALUATION OF SELF:

1. This fieldwork enabled the trainee to understand the nature of Samaritan society of
Mizoram, how the agency started from a philanthropic work from little donations
from well wishers and charities and grows to become one of the elite agencies of
Mizoram, learning that there is no too small work for the welfare of others.
2. Through this fieldwork, the trainee learned the pattern of substance abuse in which
substance abusers first drugs use is at mean age of 14 and mostly through their peers.
This shows there is a huge need of awareness among the teenagers and intervention in
the school and Sunday schools is needed.
3. The trainee also came to learn there is large number of problems related to substance
abuse physically and mentally. The trainee learned that substance abusers needs cure
of their physical health as well as their mental health.
4. The trainee through client meetings and case studies came across that there is little or
no support from the families. And there is no place where they can share their
thoughts and feelings and most of them are depressed. It increases the trainee
knowledge on how family intervention is important and that everybody needs a
friend.
5. This fieldwork increases the trainee knowledge on HIV/AIDS, particularly on High
risk groups and bridge groups.
6. This field work increase the trainee observation skills, writing skills, listening skills
and communication skills through weekly meetings, outreach, Behavioral change
communications etc.
7. On working with the staffs of the agency such as Outreach workers, peer educators
etc. who work for many years. It improves the trainee social work skills and
knowledge.
8. The trainee learned that there is still a huge need of awareness among the people. The
trainee also learned the main community views on substance abusers; they see them
as dangerous beings that are cancers to the society while the trainee with all
knowledge gains from the fieldwork started to see them as patients who need physical
and mental cure as they have no power over their addiction.

CONCLUSION:

The fact that can be withdrawn from 26 days fieldwork on the agency working along with the
substance abusers is that they are completely powerless over their drug addiction. Until and
unless there is a structured help from outside there is no chance to pull them out for their
addiction. So, there is a huge need of awareness and interventions to make in order to help
our fallen brothers and sisters. Along with the outside help, it is also important for the
substance abusers to have the will to cease their drug use, as once our father of the nation,
M.K.Gandhi has said “Strength does not come from physical capacity. It comes from an
indomitable will.”
