    1) The documentation for an ISO 14001 certification at a Libyan oil and gas company was insufficient, lacking updated environmental management reports, legal documentation of controls, and environmental impact assessments. 2) The root cause was determined to be time constraints and insufficient staff assigned to documentation tasks. 3) Corrective actions agreed upon were to gather all existing and updated records and assign a competent individual to compile the documentation according to company procedures by February 4, 2010. Preventive actions were to timely collect documentation with dedicated folders and maintain a document register also by February 4, 2010.

    Problem/Non Conformity/ Description/Justification:

    1) The documentation for ISO 14001:2204 requirements was not sufficient to cover the
    requirements for the certification such as:
     Updated records of weekly, monthly or annual reports for Wafa field environmental
    management.
     Legal documentations and evidence of EMS controls and mitigations
     Records of Environmental Impact assessment were not evident.

    Root Cause :

    Time constraint and insufficient individuals assigned to do the documentation.
