Problem/Non Conformity/Near Miss Description/Justification :
 Dumping area required reorganization as different types of waste are mixed and in
total mess.

Root Cause :
There was no proper management/ no adequate control/ failure of segregation/ open access
to the area.

Concerned Person Agreement: Problem/Non-conformity in variance with:

HSE on the behalf of the contractor. Environment protection requirements
(ISO 14001)

Agreed Corrective action and estimated completion date:

 Limit the access to the area by establishing control gate.(7th February 2010).
 Request the contractor to survey the site and re-allocate major objects (e,g. aerosol
cans, filters, etc.) to their proper location.( 24th January2010)
 FM to issue a memo to ensure that all employees commit to waste segregation policy.
HSE to support by checking and awareness to employees.( 24th January2010)
 FM to issue a memo to ensure that no one can be sending any waste to the dumping
site without authorization from HSE dept.( 24th January2010)
 Instruct the contractor (through an official memo from FM) to abide to waste
segregation and proper handling on the dumping site.( 24th January2010)
 Instruct the contractor (through an official memo from FM) to stop any burning activity
and ensure that final disposal is delivered off site.(24th January2010)

Agreed Preventive action and estimated completion date:

 HSE to schedule a weekly survey of the dumping site and report any non-
conformance.(28th January2010)
 HSE staff to observe the commitment of staff to the waste segregation and report any
non-conformance.(on going process)
