members the learning process needed to accomplish the course requisites as reflected in the syllabus.

I acknowledge that the culmination of the Architecture program is to allow the students to practically apply
all the concepts and theories learned from their previous subjects and prepare them to become a
professional and motivated architect through the Architectural Design 9 and Architectural Design 10
Thesis endeavor. With this, I hereby allow my daughter/son ________________________________ to
conduct research, interviews, ocular site inspection, analysis, and other field work and related activities
outside the campus from August 2019 to May 2020 (First and Second Semester, SY 2019-2020).

I fully understand that it is my daughter’s/son’s responsibility to provide and update me with accurate
details pertaining to his/her class activities outside the campus. I am also aware that the Thesis
Coordinator and Thesis Adviser will not accompany the students. Likewise, I also recognize that fortuitous
or unexpected incident(s) beyond the control of Thesis Coordinator, Thesis Adviser and school authorities
may still occur.
