INSURANCE COMPANY ESTIMATES:

If you have put in a claim and received an Estimate from an Insurance Company, you must present that Estimate to
Ott's Auto Body Shop LLC prior to receiving an Estimate from us. If we have done an Estimate for the damage
before you took it to the insurance company, it then makes our estimate obsolete. We will work with that Insurance
Company based on their Estimate to repair your vehicle. Therefore no Estimate will be provided from us. If you
decide to put in a claim with an Insurance Company, I agree to provide Ott's Auto Body Shop LLC with a copy of the
Insurance Companies Estimate prior to any parts being ordered or any work started to my vehicle. The shop will then
work off the Insurance Companies Estimate to repair my vehicle and any other Estimates will then be null and void.
The insurance company's estimate is the final determination.

We will not do any refunds on an insurance claim. We will follow the insurance company's determination. The
insurance company is paying for us to do he work that is stated on their estimate and supplements. We need to
follow their direction. We will make every accommodation to work with our customers when possible.

NOTE: It is the law to void the insurance companies laws and regulations. Anyone to bend these laws can be
penalized.

OTT'S AUTO BODY SHOP LLC PRELIMINARY ESTIMATES:

This Preliminary Estimate is NOT to be considered a Guaranteed Cost of Repairs. Final Cost of Repairs will not be
known untill repairs have been completed. Again, this is a PRELIMINARY ESTIMATE based on Ott's Auto Body Shop
LLC's Initial Inspection of "VISIBLE DAMAGE" to the "PRIMARY POINT OF IMPACT" to your vehicle. Once repairs have
started and visible damaged parts removed, it is possible to find additional damage. Additional damage is referred to
as 'HIDDEN DAMAGE.' We may also find damage to other areas of your vehicle that are a direct result of the
"PRIMARY POINT OF IMPACT." This type of damage is referred to as "SECONDARY DAMAGE." Any "HIDDEN or
SECONDARY DAMAGE" found will result in Additional Charges. These Additional Charges are referred to as
"SUPPLEMENTAL CHARGES". If an Insurance Company is paying for the damages to your vehicle. We will submit all
Supplemental charges to that Insurance Company for payment. If you are paying for the damages, known as "SELF
PAY'. You will be responsible for paying all Supplemental Charges.

NOTE: Please remove all items from your vehicle before leaving it for repairs. We do not want the responsibility for
any lost items.

PAYMENT TERMS:

1) Deposits are required on all jobs before work can begin. These vary. We will let you know what is required and
when.

2) All checks must be made payable to Ott's Auto Body Shop LLC

3) Supplemental Checks are to be presented to Shop for immediate deposit when received.

4) Deductibles are due upon completion of repairs.

5) No vehicle will be released until payment has been made in full.

6) If a Lein-Holders endorsement is required on an Insurance Companies check, it must be obtained prior to the
release of your vehicle.

LIKE KIND AND QUALITY TO THOSE BEING REPLACED. INCIDENTAL CHARGES SUCH AS TOWING, PROTECTIVE
CARE, CUSTODY, STORAGE, DEPRECIATION, BATTERY AND TIRE REPLACEMENT ARE NOTED WHEN APPLICABLE.

Estimate based on MOTOR CRASH ESTIMATING GUIDE and potentially other third party sources of data. Unless
otherwise noted, (a) all items are derived from the Guide DE2JC99, CCC Data Date 09/16/2020, and potentially other
third party sources of data; and (b) the parts presented are OEM-parts. OEM parts are manufactured by or for the
vehicle's Original Equipment Manufacturer (OEM) according to OEM's specifications for U.S. distribution. OEM parts
