SWORN STATEMENT

I, MARY JANE ERAZO VIRTUDAZO, of legal age, Filipino,

married, and currently employed at Justice Mubarak Al Kabeer and a
resident at Hawally, Kuwait holding a CIVIL I.D. no. #278062103191, after
having been duly sworn to in accordance with law, do hereby depose and
state that:

1. I am the same person as stated above ;

2. That I had received a sum of 2000 KD (Kuwait Currency) from my

loan in the bank of Kuwait Finance House on May 2015.

3. I have spent the loan amount in my house renovation at Asian

Heights Subdivision, Surigao City, Philippines. I have also spent the
same buying our furniture, fixtures, and house appliances.

4. That the bills/receipts of the aforementioned expenses were

unconsciously lost due to the length of time in our possession.

5. We exerted diligent efforts to locate the same but the said

receipts of the expenses could no longer be found and hence it
was lost for all intents and purposes.

6. I execute this to affidavit to attest to the truth of the foregoing

facts and for whatever legal purpose this affidavit may serve best.
