AFFIDAVIT OF LOT IDENTIFICATION

I, OLIVENA LORENO DELA PEÑA, of legal age, Filipino, married (separated in fact) to Hector
Dela Peña and a resident of Barangay Tagbay, Samal District, Island Garden City of Samal, do
hereby depose and say:

1. That I am the vendee of the Four Hundred (400 s.q.m.) square meters lot owned by Annie
Loreño, Rene Loreño and Jocelyn Loreño, a parcel of land covered by Transfer Certificate
of Title (TCT) No. 142-2019000934 containing an area of SEVEN THOUSAND TWO
HUNDRED FIFTY EIGHT (7,258) SQUARE METERS, more or less, located at Barangay
Tagbay, Samal District, Island Garden City of Samal, Davao del Norte;

2. That on November 07, 2019, we executed a Deed of Absolute Sale notarized by Atty.
Iryshell P. Villegas and recorded in her notarial register under Doc. No. 46, Page No. 10,
Book No. 08, Series of 2019;

3. That Annie Loreño, Rene Loreño and Jocelyn Loreño sold a portion of the abovementioned
parcel of land more particularly Lot No. 1040-G-1 containing an area of Four Hundred (400
s.q.m.) square meters, more or less, portion of lot: 1040-G, PSD-11-143597, more
particularly described as:
