Competency Performance Checklist

APGAR SCORING

1. Place the newborn on a warm covered surface with adequate lighting, or on the mother.
2. Evaluate the newborn exactly 60 seconds after delivery. As follows:
a. Heart rate: Use a stethoscope to listen to the newborn’s heart.
b. Award a score of 0, 1 or 2 depending on the rate or
c. palpate pulse at the junction of the umbilical cord and skin for 6 seconds.
Multiply rate by 10.
d. Now, evaluate newborn’s rspiratori effort and award a score (0,1 or2).
e. During the first 10-15 minutes irregular respiratory efforts may b as fast as 60-80 per minute.
Rhonchi, grunding, retracting and alar flare may be present for a brief period.
f. Evaluate the newborn’s muscle tone and award a score (0,1 or 2).
g. Test the newborn’s response o stimuli by suctioning the nose or drying the
newborn and award a score (0,1 or 2).
h. Evaluate the newborn’s body colour and award a score (0,1 or 2).
i. Calculate the final score after one minute and five minutes. Do not remove
the infant from the mother’s skin for these calculations.
j. Interpret the data.
3. Prevent unnecessary heat loss by minimal exposure of the newborn.
4. Repeat the evaluation according to the Apgar score five minutes later.
5. Document the Apgar scores and interventions. If any, on the newborn’s
record.
