Re: Mr.Brett Colister D.O.B. 21/12/1973

Thank you for seeing Mr.Colister whose features are consistent with diabetes mellitus type 2. Your evaluation and
management would be acknowledged.

Initially, Mr.Colister presented on 22/04/2017 with worsening tonsillitis and tiredness for which amoxicillin was
prescribed. On the subsequent two visits, he was diagnosed with a rotator cuff tear of the left shoulder and
osteoarthritis of the right knee. At that time, his examination revealed a high BMI of 30; accordingly, ibuprofen was
commenced, physiotherapy was recommended and lifestyle modifications in order to decrease weight were advised.

Three weeks ago, Mr.Colister reported having painful eyes, dizziness along with tiredness of a few-week duration,
which were probably due to orthostatic hypotension. In addition, non-adherence to the previous advice was noticed.
During the examination, there were unremarkable findings apart from the remaining high BMI. Consequently, blood
investigations were ordered.

On today's review, Mr.Colister's condition has not changed and his vision has deteriorated. Additionally, the blood
investigations illustrated an elevation of the random as well as fasting glucose levels with elevated Hba1c of 8.5%
and raised lipid profiles.

Based on the above data, diabetes mellitus type two is suspected. Thus, it would be greatly valued if you could take
over Mr.Colister's care.
