Dear Mrs. Walters

Re: Mrs. Doris White
D.O.B. 01/04/1955

Mrs. White is due to be discharged today after a successful treatment of her leg injury. Your further
management of her wound would be acknowledged.

On 25/03/2019, Mrs. White had been hospitalized due to a neglected leg injury for which she was treated
with amoxicillin, paracetamol and topical pressure therapy. However, two weeks later, her wound did not
heal and a surgical debridement was performed. At that time, her ankle brachial index was 60.

On the following month, since Mrs. White had had unrecovered wound, she underwent a skin graft with a
flap. Subsequently, her graft was healing; in addition, hyperbaric oxygen and negative pressure dressing
were used.

Today, Mrs. White's graft has healed up to 70%, and her ankle brachial index is currently 70. It should be
noted that Mrs. White has diabetes type two, ischemic heart disease, hypertension, peripheral vascular
disease along with chronic renal failure for which she is on regular hemodialysis. Furthermore, a list of her
medications including the dose is enclosed with my letter for you.

Based on the above data, I would be grateful if you could take over Mrs. White's care for dressing of her
wound.
Please contact me in case any additional clarification is required.

Yours sincerely,
Doctor
