M1 WHILE TAK
There are various pathways for the exchange of infectious
elements and irritants from the pulp to periodontium or vice
versa, leading to the development of endodontic periodontic
lesions
Pathways of Developmental Origin
(Anatomical Pathways)
Apical foramen, accessory canals/lateral
canals
Apical foramen:
Main route
Fastest route between the pulp &
periodontium
Entry and exit point of bacteria and
inflammatory byproducts resulting to
periapical pathosis

Pathways of Developmental Origin
(Anatomical Pathways)

Lateral & accessory canals

Present randomly in within the root
Another additional pathway for bacterial
byproducts

Pathways of Developmental Origin
(Anatomical Pathways)
Dentinal tubules
Pathways between pulp and PDL
(exposed dentinal tubules)

Pathways of Developmental Origin
(Anatomical Pathways)
Developmental malformations as
palatogingival grooves of maxillary incisors
Starts in central fossa going to the
cingulum, and then extend to the apical
portion
Pathways of Developmental Origin
(Anatomical Pathways)
Perforation
Usually caused by large carious lesions,
resorption, or from clinician’s error such
as RCT perforations.
Pathways of Developmental Origin
(Anatomical Pathways)
Vertical root fractures
Can produce deep periodontal pockets
and breakage of the alveolar bone
(localized)
Fracture sites are possible entry points
for irritants from the root canal to the PDL

Pathways of Developmental Origin
(Anatomical Pathways)
Root fracture following trauma
Usually caused by trauma or masticatory
forces
Portal of entry of bacteria
Pathways of Developmental Origin
(Anatomical Pathways)
Idiopathic root resorption: unknown origin
Pathways of Developmental Origin
(Anatomical Pathways)
External root resorption
From outer part going to the pulp
Pathways of Developmental Origin
(Anatomical Pathways)
Internal root resorption
From pulp going to the outside
Pathways of Developmental Origin
(Anatomical Pathways)
Loss of cementum to external irritants

LOCALIZED GENERALIZED
Pathways of Developmental Origin
(Anatomical Pathways)
Iatrogenic origin (pathways):
Exposure of dentinal tubule following root
planning
Pathways of Developmental Origin
(Anatomical Pathways)
Iatrogenic origin (pathways):
Root fractures during endodontic
procedures
