ACOMOREF 2 is the objective refractive power measurement equipment capable of
measuring the refractive powers of the eye, pupil size, as well as conducting retro
observations, accommodation micro fluctuation analysis, and addition power
measurement. This equipment is intended for use by medical doctors, orthoptists, and
optometrists at indoor (the clinic, the optometrist’s store, etc.).

In the refractive power measurement, the spherical power, cylinder, and cylinder axis are
measured by projecting light into the patient eye and receiving reflective light from the
fundus. In the pupil size measurement, the pupil sizes in lateral and vertical directions
are measured.
In the retro observations, the cloudiness in the eyeball optics system can be observed,
and binarization of the opacity degrees and measurement of the opacity range are
possible.
In the accommodation micro fluctuation analysis, measurement to check for
abnormalities in the patient’ eye condition and regulatory status is possible.

This equipment is composed of the single unit. The unit incorporates the power supply
unit and the power can be supplied to the unit by means of the power cord. The
equipment has a function to output measurement data to a computer, etc.

4-5 Accuracy check using the model eye

1. Insert the two pins of the model eye into the
mounting holes for the model eye located at
the forehead support.
2. Check that the VD indication on the LCD
monitor is 12.0.
3. Holding the Joystick, carry out alignment for
the model eye.
(For the measurement method, Refer 4-7-1 “AUTO measurement mode.”)
4. Compare the measurement values for the model eye with the values (SPH, CYL)
described on the label attached on the model eye and check that difference between
the two values falls in the range shown below.
SPH : ±0.25 D CYL : ±0.25 D
If the difference does not fall in the range shown above, check whether the
measurement process is correct by referring to 4-7-1 “AUTO measurement mode”
and conduct measurement again. In addition, Refer 7-5 “Cleaning of measurement
window” and 7-6 “Cleaning of model eye” and check for dirt. If dirt is found, conduct
cleaning. If the value does not fall in the range after conducting measures described
above, contact your dealer.

4-6 Preparation for the examinee

Clean the chin rest and forehead support with soft cloth soaked with rubbing alcohol.
(Refer to 7-4 “Cleaning of Chin rest and Forehead Support”)
When using the chin rest paper, use a new sheet of paper for each patient. (Refer 3-1
“Installation and preparation for measurement.”)
Explain the details of the measurement thoroughly to the patient in advance.
The patient is anxious inside about what is going to happen. Try to make the patient feel
relaxed as much as possible.
35
Explain the following briefly to the patient.
・ “The measurement will find out the best fit lens prescription automatically.”
・ “Hold your eye as still as possible.”
・ “Try to look at the center of the fireworks.”
Let the patient sit on the front of the equipment with both of his/her hands placed lightly
on the lap. Adjust the heights of the motorized table and the chair so that the patient can
assume a comfortable position. Let the patient put his/her forehead onto the forehead
support and place the chin completely on the chin rest. If the chin is placed insufficiently
on the chin rest, the eyelashes may stay in the optical path, influencing the
measurement.
Optical axis height mark Optical axis height mark

Adjust the height of the chin rest so that

the height of the patient’s eye matches
the standard height mark of the
equipment. When moving the chin rest
up and down, the examiner must notify
the patient of the movement and keep an
eye on the patient during the operation.

Adjust the position of the measuring unit

with the joystick so that the eye is displayed on the monitor
screen. As for vertical directions, adjust the optical axis height
mark of the equipment’s measuring unit to match the standard
height mark.
As for the horizontal directions, adjust the measuring unit
position to match the target line on the sliding table.
When the eye under test begins to appear on the screen, make
the pupil image positioned at the center of the screen. (You can
start the measurement with either side of the eye – left or right.)
Target line

・ When moving the sliding table, be careful that the table does not hit the examinee,

・ When measuring the other eye, pull the sliding table toward the examiner once, and then move it

to the other eye. Be careful not to move the sliding table as it is after the measurement because

doing so may cause the examinee’s nose to hit the equipment, resulting in injury.

4-13-3 Cutting Procedure of Printer Paper
Tear out the printer paper by holding the right edge of the paper, and cut the paper by
pulling it up to the direction as shown in the figure on the right.

・ Small amount of paper dust generated when teared off the print out paper may be
caused failure to print by adhering to the printer vent or thermal head. Clean when
necessary to avoid failure to print out.
・ Tear the print out paper with matching to the cutter direction. Do not tear to the
opposite side which will cause damage to the printer unit.

4-14 Turning off the power

At the end of the work (daily), make sure to turn OFF the power. After the power is
turned OFF, cover with the dust cover (provided) on the main body. If the equipment
will not be used for a long period, pull the power cord out of the outlet. Leaving the
power cord plugged to an outlet for a long period of time may cause a fire.

OFF
4-15 Others
4-15-1 Auto Fogging System
Auto fogging system is used to minimize the patient’s eye accommodation. When
patient’s eye matches the system requirement, it will automatically change to
fogging condition. Pleasekeep remind patient to look into the central of the fixation
chart.

68
1. Once ready for measurement, fixation chart will looks blur at patient side.
(Fogging Status)

2. Acquire the data.

3. Beep sound play once obtained the data.

4. The fixation chart remains blur at patient side.

5. Proceed to acquire the next data.

4-15-2 About Representative Data

The “Representative data” is the middle of the measurement data after sorted by the
computer.
The value with the “*” mark in the print out is the representative data. For the
example of print content, Refer 4-13-2 “Contents of printing.”
The reliability data is displayed with the representative value. The smaller the
variation the higher the reliability value (Max 10).

In cases where the variation in the Ref measurement is large, use the following points
as reference when you select one value from an individual measurement result.
・ When only SPH varies significantly.
Patient’s eye is intervened by accommodation. Restart the measurement again.
Remind patient to look at the central of fixation chart.
・ When CYL and AXIS vary
AXIS data will slightly unstable when CYL is low or <0.5D.
In other cases:
・ The eyelashes are in the area of pupil.
・ The pupil size is smaller than 2.3 mm diameter.
・ The patient has an eye disease. (Such as irregular astigmatism, opacity in the optic
media)
In these cases listed above, the reliability value tends to be low in general. When the
eyelashes or the pupil size is the cause, perform the measurement again by paying
attention to them.

4-15-3 About REF Reliability Value

The reliability value indicates the degree of variation in the measurement result. The
value is displayed when the measurements were performed three times or more and
is printed on the far right of the representative data. For the example of print content,
69
refer 4-13-2 “Contents of printing.”
・ 8 or higher: The variation is small.
・ 7 or lower: There is a variation. The handling of the measurement result requires
adequate care.
Check the unstable data under C mode measurement.

4-15-4 About Auto Power Saving

When there is no switch operation within the preset time while the measurement is
not performed, the power of this equipment automatically goes into the power saving
mode.
To perform the measurement again, press the joystick button/touch the LCD
screen/change the LR eye.
In the Menu Setting screen, the preset time for the power saving mode can be set. (→
Refer 5-10 “OTHER Setting Screen”)
* Note that the power saving mode is not activated during the measurement or in the
<SET UP> screen.

4-15-5 About Measurement of IOL Eye

This equipment can measure IOL eyes (eyes with intraocular lens) using the same
operation as that for normal eyes without any switching operations. However,
depending on the type of IOL and/or the condition of IOL eye, the reflected light from
the IOL may acts as a noise and the measurement may not be performed, the
reliability value of the measurement value may be low, or the measurement data
may vary.
By setting the Auto Quick function in advance, the measurement mode goes into the
QUICK MODE automatically when the measurement is difficult (Refer 4-12-3 “Auto
Quick Function”), and this makes the measurement easy.

4-15-6 When Measurement Data Cannot Be Obtained

When a normal measurement value is obtained, it is displayed on the main body.
However, when the measurement value cannot be obtained even if the alignment is
correct, there might be some kind of error. The handling methods for such cases are
as follows.
・ Keep the patient’s eyelashes out of the outer line of the alignment mark on the
screen. Ask patient to lift up the eyelash or widely open the eye.
・ Blinking during the measurement will restart the measurement automatically.
・ SPH measurement is out of range (-20D to +23D) or CYL measurement is out of
range (+- 0~12D).
・ The measurement cannot be performed when <S+C> is outside the measurement
range from -20D to +23D or <C> is outside the measurement ranges from 0D to
-12D and from 0D to +12D.
70
・ When measuring the patient eye with a contact lens:
Normally the measurement value can be obtained. However, if the contact lens does
not fit completely, the correct value cannot be obtained. The measurement cannot
be performed when the contact lens is dirty or damaged. ,
・ When measuring the patient eye with glasses:
The measurement value can be obtained even if the lens is slightly tilted. The
correct value cannot be obtained if the lens is tilted too much. The measurement
may not be performed when the reflected light on the lens surface goes into the
measurement window or the lens transmission is low in case of a color lens.

6. Connection to External Devices
The connector is used for the interfaces in conformity with EIA RS-232C. The
measurement results can be transmitted to external devices, such as a PC etc. via this
connector.

6-1 Connection with Remote Vision RV-II

Before starting the subjective examination, the REF data can be transmitted
automatically for faster measurement.

6-1-1 Wireless Transmission to RV-II

When you are using RV-II, the data can be transmitted wirelessly using infrared. To use
wireless transmission, set the “IR Setting” in 5-3-2 “Data Signal Setting Screen” to “RT.”
Place RV-II face with this unit without obstacle in between. If data cannot transmit
under the recommended configuration distance, check and adjust the distance or range
for both units.

6-1-2 Cable Transmission to RV-II

(Note: This function will be implemented in RV-II software version 4.00.01.)
To connect to the RV-II using a cable, use a commercially available RS-232C cable (9-pin
female to 9-pin male). Remove the RS-232C cap from the main body and connect the
RS-232C cable. After the RS-232C cable is removed, attach the RS-232C cap again for the
protection against electric shock. To output to RV-II, set “FORMAT” on the Data Signal
Setting Screen to “RT.”(→ Refer 5-3-2 “Data Signal Setting Screen”)

・ The operator must not touch the pin(s) of RS-232C and the patient
simultaneously.
This will cause electric shock.

6-2 Connection with PC

The measurement results can be output to a PC etc.
By creating an appropriate program, recording and statistical processing of the data can
be performed efficiently.
88
6-2-1 Connection with PC using USB
Use a USB cable (A-B type connector) to connect the equipment with a PC.
Connect the B type connector to the equipment.
To output to a PC, set “PORT” on the Data Signal Setting Screen to “USB.” (→ Refer
5-3-2 “Data Signal Setting Screen”) and “bit/sec” / “Stop Bit” accordingly (Refer to
5-3-2 “Data Signal Setting Screen”)

6-2-2 Connection using RS-232C

Use a RS-232C cable (9-pin female).
Remove the RS-232C cap from the main body.
Connect the 9-pin female connector to the equipment.
To output to a PC, set “PORT” on the “Data Signal Setting Screen” to “D-SUB.” (→
Refer 5-3-2 “Data Signal Setting Screen”) and “Bit/Sec” / “Stop bit” accordingly.
(Refer to 5-3-2 “Data Signal Setting Screen”)
After the RS-232C cable is removed, attach the RS-232C cap again for the protection
against electric shock.

・ The operator must not touch the pin(s) of RS-232C and the subject
simultaneously.
This will cause electric shock.
For the details on the connection with a PC, contact the dealer where you purchased
the equipment.

89
7. Maintenance
This device does not require periodic calibration.
7-1 Check of Measurement Accuracy
Check the measurement accuracy every time before using the equipment by using the
model eye provided. For the measurement method using the model eye and the accuracy
check method, Refer 4-5 “Accuracy check using the model eye”. If the value does not fall
in the range after conducting measures described in 4-5 “Accuracy check using the model
eye”, contact your dealer.

7-2 Change of Printer Paper

When the end of the printer paper approaches, red lines appear on the both sides of the
paper. When the red lines appear on the paper, replace it with a new one. (Please use the
printer papers specified by us.)
For the replacement method of the paper, Refer 3-2 “Preparing Printer Paper”

7-3 Replacement of Fuse

Caution

・ Before checking or replacing the fuses, make sure to turn OFF the power switch
and pull the power cable out of the outlet.
・ Use the fuse shown below for the both fuses. Do not use other fuses.
To order the spare fuses, contact the dealer where you purchased this equipment.
Manufacturer: Littelfuse
Time lag fuse: φ5 x 20 mm
250V/T1.6AH (021501.6XP)

When the POWER lamp and/or fixation chart do not light up and the LCD screen
does not brighten up even if the power switch is turned ON (“|” side), the fuse may
be blown out.
Disengage the right and left claws of the fuse holder using the tip of a fine flat blade
screwdriver and pull out the holder.
Pull the fuses (2) out from the holder. Check if the fuses are blown out and replace
them if necessary.

90
The spare fuses are located in the spare fuse holder inside the printer cover.
(→ Refer 2-1 “System configuration”)

7-4 Cleaning of Chin rest and Forehead Support

Cleaning should be done before the measurement. Clean the chin rest and forehead
support with soft cloth, etc. soaked with rubbing alcohol.
ex) “Schnelldesinfektion”, “Sixtan® Desinfektion” : They are registered in the VAH list.
When using the chin rest paper, use a new sheet for each patient.
For the replacement method of the chin rest paper, Refer 3-1 “Installation.”

7-5 Cleaning of Measurement Window

A dust-proof glass is mounted in the measurement window. When dust is seen from the
patient side, use the blower (provided) to clean the dust.
When the dust cannot be removed after that, wipe the glass with lens cleaning liquid or
anhydrous alcohol (commercial item).

・The dust-proof glass is thin and fragile, so wipe it gently without applying
excessive force.

7-6 Cleaning of Model eye

When measuring the model eye, the correct value cannot be obtained if the lens surface
of the model eye is dirty or if dust, fingerprint, etc. is attached to the surface. In such
cases, use a gauze with a small amount of lens cleaning liquid or anhydrous alcohol
(commercial item) and wipe the lens surface without damaging it. (Do not use a
handkerchief or absorbent cotton.)

・Damage on the surface of the model eye may have influence on measurement
accuracy. To prevent the damage of the model eye, do not hit it with a hard
object or throw it.

91
7-7 Cleaning of Unit (Exterior)
When the exterior becomes dirty, wipe it with soft cloth. For severe dirt, use a soak gauze
(dilate with mild detergent) to wipe it gently.
We recommend the use of the dust cover provided after using the equipment for the
prevention of fouling, such as dust.

・Do not use any organic solvent (alcohol, ether, thinner, etc.). Discoloration
and/or damage may be caused.
