Subject: Letter of Explanation

Warmest Greetings!

This Letter is to explain the surrounding circumstances relative to the delayed registration of the
facts of my birth in my Certificate of Live Birth before the Office of the Municipal Registrar of
Tadian, Mountain Province, Philippines.

I was born of Spouses Andrew M. Palangeo and Catalina T. Palangeo, on June 14, 1986 at
Bunga, Tadian, Mountain Province, Philippines.

According to my parents, I was born in our house because our barangay is located in a remote
place where no road is available going to the hospital in our town. So that at the time of my
delivery, my mother was assisted by a Barangay Health Worker (BHW).

Moreover, the facts of my birth were also taken and thereafter prepared by the barangay health
worker (BHW) to be registered with the Municipal Civil Registrar in our town. Believing in
good faith that the BHW has caused the registration of the facts of my birth at the time of my
birth or immediately thereafter, my parents did not attempt to register the same with the said
Municipal Civil Registrar. It is only later or in the year 1997 that my parents have discovered
that the facts of my birth were not registered when they tried to secure a copy thereof for
personal consumption;

Consequently immediately, my mother caused the registration of the facts of my birth on

October 6, 1997 with the Municipal Civil Registrar of Tadian, Mountain Province;

For this reason, my parents are really in good faith in the delayed registration of my birth
believing in good faith that the BHW had already caused the registration of the same;

It is hoped that you are enlightened by this letter and seriously consider the matters as stated
above.
