AFFIDAVIT 

I, Nathaniel Tac Al Cassion, Filipino, of legal age, married, and a resident of Barangay

Milibili, Roxas City, Capiz, Philippines, after being duly sworn in accordance with law, depose
and state that:

1. I am presently employed as Teacher I at Congressman Ramon A. Arnaldo High School,

Banica, Roxas City;

2. On July 4, 2018, when I went to school for duty, I noticed that the lock of the room of Sir
Larimar V. Araw-Araw SHS-Teacher II of the same school and the two jalousies of the
said room’s window were destroyed. Evidently, Sir Araw-araw’s room was forcibly
opened. As reflected in the police blotter certification, the date of the incident in question
is July 4, 2018;

3. Resultantly, after the inventory of the items originally placed/stored/kept inside the
aforesaid room, the following were noted to be missing or stolen:

4. Despite diligent search and efforts to locate those items/tolls/equipments, the same
proved futile. Consequently, the incident was reported to the Philippine National Police,
Roxas City.

5. I am executing this Affidavit to attest to the truthfulness of the foregoing.

Libas, Roxas City, Capiz, Philippines, after being duly sworn in accordance with law, depose
and state that:

1. I am presently employed as SHS-Teacher II at Congressman Ramon A. Arnaldo High

School, Banica, Roxas City;

2. On July 4, 2018, when I went to school for duty, I noticed that the lock of my room and
the two jalousies of the said room’s window were destroyed. Evidently, my room was

5. In consonance with the Commission on Audit Guidelines dated August 10, 2017, with
the subject: “Documentary Requirements on Request for Relief from Money and
Property Accountability in case of Loss of Government Property” which states that: “ 2.2
xxx xxxx In case it is not possible to obtain the statement of two disinterested persons
individuals and only one is available xxx, such fact shall be set forth in the affidavit of
the person requesting relief, giving the reasons thereof;”, I am attaching the affidavit of
my fellow faculty Sir Nathaniel Tac Al Cassion.

6. My other intended witness in the person of Mrs. Carmen B. Alabot, is not presently
available today as she has very important school matters to attend to making it physically
impossible for her to appear before an officer authorized to solemnize oath.

7. I am executing this affidavit to attest to the truthfulness of the foregoing.
