PAKYAW (TAKAY) CONTRACT

KNOW ALL BY MEN BY THESE PRESENTS:

This Contract “Pakyaw –Takay” is made and entered into by and between the Department of
Environment and Natural Resources, represented by
________________________________________ hereinafter referred to as the “FIRST
PARTY”

-and-

________________________, of legal age, with residence and postal address at

NHA,Brgy. Dahican, City of Mati , Davao Oriental, hereinafter referred to as the “SECOND
PARTY”,

-WITNESSETH-

That the FIRST PARTY is in need for the services of the SECOND PARTY to perform the
painting/welding and masonry works at PENRO RXI-2,Gov’t. Center, Dahican, City of Mati. Scope
of works for the ___________________________________________; are as follows;

Water line :

 Installation of new waterline system of PENRO office building ;

 Concreting of the frontage of Records building with canal outlet;

1. That both parties agreed that payments of services rendered be made in full amounting to
Twenty Five Thousand pesos (25,000.00), upon submission of completion report duly
certified by the concerned Section Chief/TWG;

2. That the Second Party has agreed to start on the 19th day of December 2018; and

3. That the Second Party has agreed to accomplish activities stipulated in the contract on or
before the 31st day of December 2018;

IN WITNESS HEREOF, parties hereto have affixed their signature below this 20 th day of December
2018, City of Mati, Davao Oriental.
