         The nurse in the postpartum unit was caring for Margarite Sanchez who gave
birth to a healthy boy 5 hours ago. Both she and her son are stable. She breastfed her
son for 15 minutes after the birth. You notice that she is lightly touching the top of her
newborn’s head with her fingertips. She comments that she does not feel comfortable
holding her baby close to her body for breastfeeding.

        The next day you are again assigned to care for the Sanchez family. Mom and
baby are stable. José, Margarite’s husband, is present during the shift. Margarite and
José voice concern about integrating their newborn into the family.

1. Discuss your nursing actions that are based on your knowledge of maternal
touch. 
In the above-mentioned case study, promoting early contact between the mother and
the infant is crucial in initiating their relationship. In this regard, nurses play an important
role in assisting both the mother and the infant in the attachment process by promoting
and encouraging mother-infant bonding. These nursing actions include promoting
skin-to-skin contact by placing the infant naked or wearing a nappy, prone, and upright
onto the mother's bare chest. This enables the mothers to have physical contact with
their infant, and enhanced mother-infant attachment, and also contributes to early
development. Encouraging the mother to breastfeed is also important in fostering
maternal touch and attachment as it provides both of them with comfort, a feeling of
physical closeness while also encouraging the mother to take a greater role in the
infant's vitality. Although initially, the client Mrs. Sanchez verbalizes being
uncomfortable with holding the baby too close to her body for breastfeeding, it is
important to note that the mother is still in the process of claiming or binding in specific
features as she identifies the infant as her own. Over time, as the mother explores the
infant with her fingertips, her maternal behavior soon begins to change as she goes on
to discover her baby. 
2. List the maternal phase and expected maternal behaviors for this period of
time. 
In the case study, Mrs. Sanchez is categorized to be under the taking in period of the
maternal phase since she is still within the first two days of delivery. Maternal behaviors
include the following:
  uncertainty of how she could care for the newborn
 extreme tiredness that follows childbirth
 dependent on her healthcare provider or support person with some of the daily
tasks and decision-making
 the woman is passive
 prefers to talk about her experiences during labor and birth and also her
pregnancy

4. Discuss your nursing actions that reflect an understanding of the couple's
transition to parenthood.

For relatively new parents, the transition towards parenthood and a caregiving role can
often be viewed as a crisis. It is a time of disequilibrium and disorder as the couple
themselves take on a less carefree lifestyle, which may even render some coping
methods with being ineffective at times. They may even be so distressed to the point
that both individuals are unable to support each other. Due to the fact that men typically
identify their spouses as their primary or only source of support, the transition can be
comparatively more challenging for them. Men often feel deprived when the mothers,
who are also experiencing stress, cannot provide their usual support level.

For this reason, many parents are unprepared for the strong emotions such as
helplessness, inadequacy, and anger when it comes to dealing with a crying infant.
Likewise, parenthood also allows adults the opportunity to develop and show a warm,
selfless, and caring version of themselves that may not be expressed in different adult
roles. In this regard, a nurse has the responsibility of helping their clients ease into the
transition of parenthood by promoting the childbearing family's integrity. Here, the nurse
prepares the clients for the expected role changes involved in becoming a parent as
well as preparing them for the responsibilities of parenthood. By providing them with this
information, the nurse is able to facilitate the growth of both the mother and her partner;
the nurse enables the two of them to provide for their own needs and address their
responsibilities, thus reinforcing positive parenting behaviors.

The nurse should also provide lactation counseling and correct the mother's
misconceptions and inaccuracies concerning breastfeeding. Here, the nurse should
evaluate the parents' understanding of the feeding process by assessing and assisting
in the feeding techniques and demonstrating ways on how to increase breastmilk
supply. By providing the parents with encouragement, education, and support, as well
as assisting in the maintenance and establishment of successful breastfeeding, the
parents become more capable of addressing the newborn's needs at this particular
developmental level. Furthermore, the nurse should implement health teachings
pertaining to the parenting skills needed to care for the newborn. This nursing action

involves imparting instructions on the nurturing and physical care needed by the infant
in their first year of life. Here the nurse provides anticipatory guidance regarding the
infant's developmental stages in life as well as determining the parents’ readiness and
ability to learn about infant care.
It is also important for the nurse to praise parents for their participation in the child's
care and tell them that they are giving good care to the child. By demonstrating ways in
which the parent can stimulate an infant's development, discussing the infant's
capabilities for interaction, and demonstrating quieting techniques, the nurse is able to
promote and reinforce a more positive parent-child relationship and also establish
positive parenting behaviors. Lastly, the nurse should be involved in identifying risks
and implementing priority strategies to prevent parenting problems in the future. It
includes reviewing the prenatal history for factors that predispose individuals or family to
complications and also planning for risk-reduction activities in collaboration with
individuals or family.
It is important to note that while some parents may view this advice from a healthcare
professional as a form of encouragement and support, others may see it as a reflection
of their own inadequacies, and this criticism, whether imagined or real, affects the
parents' abilities to care for the infant and can be detrimental in the long run. Thus, we,
as nurses, must always be mindful of the client's norms, beliefs, and values and offer
appropriate nursing care and advice that are well within their cultural boundaries in
order for them to provide the infant with a safe environment and establish a positive
relationship with the child.

5. Discuss specific strategies to decrease sibling rivalry. 


Childhood jealousy and sibling rivalry, which arises when a baby sister or brother is
born, are problems within the family that need to be resolved in a tactful and delicate
manner. It is because the actions and emotional expressions of the jealous child may
lead to behaviors that may be detrimental to the physical, emotional, and developmental
wellbeing of the most vulnerable member of the family, which is the newborn. In order to
address these grievances, the nurse needs to conduct a thorough assessment of the
various social and cultural factors that form the parents'parents' preconceptions
regarding parenting and also their attitudes towards their interactions with the jealous
child which may have contributed towards an aggravated response. It is important that
these assessments and the identification of these factors are done in a manner that

acknowledges racial and ethnic differences as well as conducting the entire process
with warmth, respect, and professional courtesy. Afterwards, the nurse should provide a
safe environment for both the child and the parents where they are given an opportunity
to voice out and express their feelings, goals and personal needs without being judged
or compared to others. Having them acknowledge each other’s feelings supports both
the child and the parents in meeting their own needs while also reinforcing positive
behaviors which increases one's feeling of adequacy. The nurse should also promote a
positive child-parent relationship by implementing methods and teaching the parents to
reduce conflict by being consistent in their approach towards the child's behavior and
needs, which involves avoiding siding with the child or other parents in a conflict
scenario. Lastly, the nurse should educate the clients regarding the developmental
tasks for both the child and parents, difference in developmental level between child
and parents, and appropriate tasks for age levels. Doing so provides information that
assists parents in responding realistically and appropriately to the child's needs at
different age levels. The parents must realize that the regression in habits and
behaviors in the older child is a typical reaction and that he needs love and attention at
this time. It is a method of seeking attention; thus, parents need to distribute their
attention equitably.
