1. I am applying for Teacher 1 position at that I understood well the DepEd Order no.7, s. 2015
(Hiring Guidelines for Teacher 1 position)

2. I have submitted the following documents:

a) A written application letter
b) CSC Form 212 (Revised 2005) in two copies with the latest 2x2 ID picture
c) Certified photocopy of PRC professional identification card or a PRC certification showing
the teacher’s name and other information recorded on the PRC office.
d) Certified photocopy of ratings obtained in the Licensure Examination for Teachers (LET)/
Professional Board Examination for Teachers (PBET)
e) Certified copy of transcript of Records with General Weight Average (GWA)
f) Certificate of specialized training
g) Certified copy of the Voter’s ID and/or any proof of residency as deemed acceptable by thE
School Screening Committee
h) NBI Clearance
i) Certification from the Principal stating that the applicant is a bona fide resident of the
Barangay where the school is located.

3. I am executing this Certification to attest to the authenticity and veracity of all documents
submitted.

4. I understand that any violation will automatically disqualify me from the selection process.

5. By executing this Omnibus Certification of Authenticity and Veracity of Documents, I hereby

authorize the Department of Education- Division of Bukidnon, through the School Head to
verify the authenticity of the above mentioned documents.
