Return to School Questionnaire Instructions 1. Organize a team of staff that will be making parent phone calls using the questionnaire on page two. Please practice with the staff to make sure they are comfortable responding to any roadblocks they may come across with parents. 2. Obtaining information may require multiple attempts to make contact. It is critical that we obtain this information for organizational purposes. 3. Some contacts may require translation services, so ensure that you have someone available to perform this function (If you have no one available, please contact the Central Office for assistance). 4, What are we trying to leam from each question? 4 Question 1 - Students who will be remaining in distance learning, b. Question 2 - Students who will be attending school when we open with a hybrid model. ©. Question 3 - Students who will only be attending school when we open with full in-person instruction. 5. Populate the “Return to School Spreadsheet” with “Y” or “N” responses in the appropriate column. 6. Ifthe family has more than one student in your school, enter responses for all students from that family separately for each student,Return to School Questionnaire Opening Statement: _— School is beginning the process of retuming students to school and I have some questions fo ask you. Keep in mind that COVID restrictions will be in place in all situations (Distancing and Masks); Question #1: Will (student name) remain in full distance (online) learning when school returns to in-person classes if the distance leaming is provided by ECSD staff? (Yes or No) 1f"Yes” for question #1, end the questionnaire, Clarifying Statement, “So just to double check, your child will be staying in full-distance learning if we return to in-person classes” If “No” for question #1, continue on to Questions 2 and 3, Question #2: ‘Will you be sending (student name) back to school once the “hybrid model” (Briefly explain your school’s hybrid model to parent) starts? (Yes or No) Question #3 Will you be sending (student name) back to school once there is full daily in-person school?
