Special Power of Attorney

KNOW ALL MEN BY THESE PRESENTS:

I, ____________, of legal age, Filipino Citizen, single, and a resident of No. ____________, Alapang, La
Trinidad, Benguet, do hereby name, constitute and appoint, ________________, likewise of legal age, Filipino
Citizen, single and a resident of Lower Bicutan, Taguig City, to be my true and lawful attorney-in-fact, for me and in
my name to do and perform the following acts and things to wit:

1) To process the closure of my BIR registration with Taguig branch, more specifically BIR RDO No. 44,
Taguig City and transfer said registration to BIR RDO No. 008 of Baguio City;

2) To submit BIR form 1905 and other documents necessary for the transfer of my registration from RDO
44 to RDO 008;

3) To make, sign, execute and deliver contracts, documents, agreements, and other writing whatsoever
nature, kind and description, with any third persons, natural or juridical, for the accomplishment of the
above-mentioned purpose; and to perform such other things that are deemed necessary to carry into
effect the foregoing authority.

HEREBY GRANTING AND GIVING unto my said attorney-in-fact FULL POWER AND
AUTHORITY to do and perform every act and thing whatever requisite and necessary to be done in and about the
premises, as fully to all intents and purposes as l might do or could do if personally present, and HEREBY
RATIFYING AND CONFIRMING all that my attorney-in-fact shall lawfully do or cause to be done by virtue of
these presents.
