JOINT AFFIDAVIT OF COHABITATION

We, RAFAELA ARTISTA and CLINT DELACRUZ, both of legal

ages, Filipino citizens, singles, and residents of Palo, Leyte, after being first
sworn according to law, hereby depose and state: THAT---

1. We are common law husband and wife respectively;

2. We have been living together as common law husband and wife

since 2004;

3. Of said relationship, we were gifted with one child, Harith

Delacruz;

4. I Lately, we have decided to get married under exceptional

character.

5. We are executing this Joint affidavit to attest to the truth of the

foregoing facts, for whatever purpose this may legally serve and in support
of our desire to get married under exceptional character.
