SUBJECT : Spot Report Re: Shooting Incident

DATE : November 4, 2018

1. References:
a. SMS dated November 4, 2018 with same subject as above;
b. PNP Standard Operating Procedure.

2. Please be informed that on or about 7:30 in the evening of November 4, 2018

a telephone call received from the concern Citizen that there was shooting incident
transpired at Brgy. Tejeros Convention, Tanza, Cavite at about 7:00 AM of same date.

. 3. Initial investigation conducted disclosed that the victim identified one

Christopher Martinez y Tan, 22 years old, born on April 2, 1998 , male, single, jobless,
native of San Luiz, Laguna and presently residing at # 24 Brgy. Tejeros Convention,
Tanza, Cavite was on his way home along the road of Brgy. Tejeros Convention, Tanza,
Cavite when one unidentified person shot the victim several times and fled on foot after
the incident. Victim was rush to Divine Grace Medical Center Brgy. Tejero, Gen. Trias,
Cavite but pronounced dead on-arrival (DOA) by the attending Physician Dr. Anthony
Lee at about 7:45 AM, Further, victim sustained multiple gunshot wound on the different
part of his body. SOCO team was requested to process the crime scene and recovered
five (5) pieces of empty shell and two (2) deformed slug. Cadaver of the victim was
brought to Josue Funeral home for Autopsy.
