Coronary artery atherosclerosis

Anatomic Summary:

I. Fifty-five year-old man found deceased on the floor of another


person's residence.

II. Autopsy findings.


A. Coronary artery atherosclerosis.
1. 30-60% stenosis of the left anterior descending
coronary artery.
2. 10-20% stenos is of the left circumflex coronary
artery.
3. 70-95% stenosis of the right coronary artery.
B. Pulmonary congestion and edema.
c. Cerebral edema.
o. Testicular edema, bilateral.
E. Subcutaneous hemorrhage and scar tissue of the right
antecubital fossa.

CIRCUMSTANCES:

Please see coroner investigator's ~eport.

IDENTIFYING FEATURES:
'
The body is identified by coroner t~gs, 2019-00170, Timothy Dean.
No surgical scars are identified. Multiple tattoos are present;
representative tattoos include: what appear to be foreign-language
characters involving the right upper chest, multiple tattoos of the
left arm and forearm, and letters of the upper back which appear to
be "TM D".

CLOTHING:

The decedent's clothing has been removed prior to autopsy, and the
clothing is examined separately. The clothing consists of two pairs
of white underwear, one of which has "PROPERTY OF NATE" written on
the rear inside waistband and a pair of gray and red jockstrap like
underwear. Flexible plastic material is also present and includes
two pieces of relatively uncolored flexible plastic and a flexible
blue plastic ring.

EVIDENCE OF THERAPEUTIC INTERVENTION:

The following findings include only those present at the time of


autopsy and do not include medical therapeutic devices removed
prior to autopsy. At the time of autopsy, evidence of recent
medical therapeutic intervention includes red abrasions of the
anterior chest and a right leg/tibial puncture site. Organ and
tissue procurement have not been performed.

EXTERNAL EXAMINATION:

The body is identified by coroner tags and ankle band and is that
of an unembalmed, refrigerated adult Black man who appears about
the reported age of 55 years. Based on coroner form #1 data, the

body is 242 pounds and 77 inches in length. Rigor mortis is


minimal. Livor mortis is red-purplcl, posterior, and fixed.

The head is normocephalic, and the scalp is covered by short dark


brown-black hair which demonstrates frontotemporal balding. Facial
hair consists of a mustache and sh~rt stubble in the beard
distribution. Examination of the eyes reveals irides that appear
brown and sclerae that are anicteric. There are no petechial
hemorrhages of the conjunctivae of the eyelids or sclerae. The
oronasal passages are unobstructed. Frenulae and oral mucosae are
intact. Upper and lower teeth are m,ostly present and atraumatic.
Manipulation of the neck demonstrates no abnormal mobility or
crepitus. !
There is no chest deformity. The abdomen is not distended.
Genitalia are those of an adult man, and the penis appears
circumcised. The external genitalia are without trauma or lesions.
Examination of the anus reveals no hemorrhage or trauma. The
extremities show no prominent edema, joint deformity, or abnormal
mobility.

The right antecubital fossa demonstrates the presence of a scar;


incision of this scar demonstrates a small amount of subcutaneous
hemorrhage as well as a small amount of subcutaneous scar tissue.

INITIAL INCISION:

The body cavities are entered through the standard coronal and Y-
shaped incisions.

NECK:

And anterior neck dissection is performed: there is no hemorrhage


of the superficial fascia, investing fascia, strap muscles, carotid
sheaths, visceral fascia, thyroid gland, or throat organs.
Following anterior neck dissection, the neck organs are removed en
bloc with the tongue. The gingivae and oral mucosae are without
trauma or lesions. The tongue, when sectioned, shows no trauma. No
fluid or foreign material is present in the mouth, pharynx, larynx,
or trachea. There is no prominent edema of the larynx or

epiglottis. Hyoid bone and larynx ~re intact and without fracture.
The right styloid process is significantly elongated and extends
almost to the level of the hyoid bdne.

There are no cervical
prevertebral fascial hemorrhages. No cervical vertebral fractures
are identified. :

CHEST/ABDOMINAL CAVITY:

Soft tissues of the thoracic and abdominal walls are well-


preserved. The subcutaneous fat of the thoracic wall is 1.0 cm in
thickness, and the subcutaneous fat' of the abdominal wall is 3. 2 cm
in thickness. No free blood or fluid is present within either
pleural cavity. There are no pleural adhesions. The parietal
pleurae are smooth and intact.

No abdominal adhesions are present. Ascites is not present. No free


blood is present within the peritoneal cavity. There is no evidence
of previous abdominal surgery. The overall general arrangement of
the abdominal organs is unremarkable, and none are absent. There is
no overt evidence of peritonitis.

SYSTEMIC AND ORGAN REVIEW

The following observations are limited to findings other than


injuries or trauma, if described above.

MUSCULOSKELETAL SYSTEM:

No overt, non-traumatic abnormalities of the bony framework or


musculature are identified.

CARDIOVASCULAR SYSTEM:

The aorta is elastic and of even caliber throughout, with vessels


distributed normally from it. The aorta demonstrates minimal
atherosclerosis. The proximal portions of the major branches of the
aorta are unremarkable. There is no tortuosity or widening of the

thoracic segment. There is no dilatation of the lower abdominal


segment. No aneurysm is present.

The pericardial sac contains a small amount of serous fluid. The


heart is 500 grams and has a vaguely globular silhouette. Coronary
ostia are widely patent, and the coronary artery distribution is
right dominant. In situ serial sectioning of the coronary arteries
demonstrates patchy, predominantly ~on-calcified atherosclerosis
resulting in up to 30-60% stenosis ~f the left anterior descending
coronary artery, up to 10-20% stenosis of the left circumflex
coronary artery, and up to 70-95% stenosis of the right coronary
artery. No intravascular coronary artery thrombi are identified.
The valves are thin, leafy, and competent. No cardiac valve
vegetations are identified. Circumferences of the valve rings are:
tricuspid 14.1 cm, pulmonic 9.0 cm, mitral 13.4 cm, and aortic 8.7
cm.

The myocardium is mildly flaccid. With respect to direction of


flow, the overall chamber configuration is unremarkable, and the
chambers are without mural thrombosis. The right ventricle is 0.4
cm in thickness, the interventricular septum is 1.5 cm in
thickness, and the left ventricle is 1.8 cm in thickness. There is
no discoloration of the endocardium. There is no abnormality of the
apices of the papillary musculature. No infarcts or focal lesions
of the myocardium are identified. There are no defects of the
septae. The great vessels enter and leave in a normal fashion. The
ductus arteriosus is obliterated. The blood within the heart and
large blood vessels is a mixture of liquid and clotted blood.

RESPIRATORY SYSTEM:

The visceral pleurae are smooth and intact. The right lung is 950
grams, and the left lung is 800 grams. No significant fluid or
foreign material is present within the grossly visible
bronchopulmonary tree. The respiratory mucosa appears unremarkable.
Hilar lymph nodes are unremarkable. Sectioning of the lungs reveals
parenchyma that is pink-red to red-purple, and the cut surfaces
exude a small amount of blood and a moderate amount of watery
fluid. No regions of consolidation are identified. The pulmonary
vasculature is without thromboembolism. There is no evidence of
prior pulmonary infarction.

GASTROINTESTINAL SYSTEM:

The esophagus is intact throughout. No dilated blood vessels are


visualized at the distal end of the esophagus. The stomach is
minimally distended, and it contains approximately 20 mL of brown
liquid (specific types of food are not recognized). No tablets,
capsules, or portions thereof are identified within the stomach or
stomach contents. The gastric mucosa is unremarkable. The small
intestine and colon are opened along their entire length and
examined. The small intestine contains a small amount of tan semi-
liquid stool. The large intestine contains a moderate amount of
brown semi-liquid to semi-solid stool. The distal portion of the
rectum demonstrates a small amount of slippery translucent material
on the mucosal surface. The appendix is unremarkable. The pancreas
occupies a normal position and is without trauma or hemorrhage. The
parenchyma is lobular and has an unremarkable consistency.

HEPATOBILIARY SYSTEM:

The liver is 1850 grams and red-brown. The capsule is intact and
the liver has an overall unremarkable gross architecture.
Sectioning of the liver demonstrates smooth cut surfaces and an
unremarkable parenchymal consistency. No fibrosis is identified
grossly. The gallbladder has a thin and pliable wall. It contains a
moderate amount of unremarkable brown bile and no calculi.
Periportal lymph nodes are unremarkable.

URINARY SYSTEM:

The kidneys are normally situated, and the capsules separate with
ease to reveal smooth and glistening cortical surfaces. The right
kidney is 175 grams, and the left kidney is 175 grams. Sectioning
of the kidneys reveals preserved corticomedullary demarcations and
otherwise unremarkable parenchyma. Peripelvic fat is not increased.
The ureters are without dilatation or obstruction. The urinary
bladder is mildly distended; it contains 40 mL of light yellow
urine. The urine is tested by the dipstick method and does not
reveal a significant amount of glucose.

GENITAL SYSTEM: '

The prostate gland is unremarkable,I without enlargement or


nodularity. Both testes are within the scrotum and without trauma;
both testes demonstrate mild-moderate edema. The skin of the penis
is reflected, and the penis is partially dissected; no puncture
site or hemorrhage is identified, tiut the distal end of the penis
is mildly edematous. !

HEMOLYMPHATIC SYSTEM:

The spleen is 250 grams and has an intact capsule. The parenchyma
is red-purple and has an unremarkable consistency. No increased
follicular pattern is identified. Lymph nodes throughout the body
are small and inconspicuous. The bone marrow of the rib is
unremarkable. The thymus is involuted.

ENDOCRINE SYSTEM:

The pituitary gland, thyroid gland, and adrenal glands are


unremarkable. Parathyroid glands are not identified.

SPECIAL SENSES:

The eyes, middle ear, and inner ear are not dissected.

HEAD AND CENTRAL NERVOUS SYSTEM:

There is no subcutaneous, subgaleal, or subperiosteal hemorrhage of


the scalp. The dura mater is removed, showing no fractures of the
calvarium or base of the skull. There are no tears of the dura
mater. There is no epidural, subdural, or subarachnoid hemorrhage.

The brain is 1275 grams. The leptomeninges are thin and


transparent. A mildly flattened convolutionary pattern is observed.
Coronal sectioning demonstrates a uniformity of cortical gray
thickness. The cerebral hemispheres are symmetric. There is no
softening, discoloration, or hemorrhage of the white matter. The

basal ganglia are intact. Anatomic1landmarks are preserved.


Cerebral contusions are not identified. The ventricular system
appears moderately compressed. Pons and medulla are unremarkable.
Bilateral uncal and cerebellar tonsillar herniation is present.
Vessels at the base the brain have an unremarkable distribution and
are without aneurysm. The cerebral arteries demonstrate no
significant arteriosclerosis.

SPINAL CORD:

The entire cord is not dissected. The superior portion of the


cervical cord is examined through the foramen magnum and is
unremarkable. The spinal fluid is clear.

RETAINED TISSUE:

Representative sections from various organs are preserved in two


storage jars containing 10% formalin.

TISSUE SECTIONS FOR MICROSCOPIC EVALUATION:

1. Heart.
2. Lungs.
3. Liver, gallbladder, kidneys, and spleen.
4. Adrenal glands.
5. Right frontal lobe of the brain.
6. Right basal ganglia.
7. Right hippocampus.
8. Left hippocampus.
9. Substantia nigra.
10.Cerebellum, dura mater, and thyroid gland.
11.Subcutaneous hemorrhage of the right antecubital fossa.
12.Pancreas.

TOXICOLOGY:

Heart blood is collected in a vacuum-intact EDTA purple top tube


and submitted to the lab as a typing specimen. Heart blood, femoral

blood, bile, liver tissue, urine, Jtomach contents, and vitreous


humor are collected and submitted tjo the lab. A comprehensive
toxicology screen is requested.

EVIDENCE:

No physical evidence is acquired frpm the performance of this


autopsy.

PHOTOGRAPHY:

Seventeen scene photographs are reviewed prior to the performance


of this autopsy. Photographs obtained during the autopsy include
photos of the neck dissection and right antecubital fossa
subcutaneous hemorrhage.

RADIOGRAPHY:

Eight radiographs are reviewed. The radiographs are unremarkable.

WITNESSES:

Sergeant Cardella of the Los Angeles Sheriff's Department is


present for this autopsy.

DIAGRAMS AND FORMS USED:

Diagrams/forms #16 and 20 are used during the performance of this


autopsy. Coroner diagrams are not intended to be facsimiles nor are
they drawn to scale.

SUMMARY AND OPINION:

This 55 year-old man, Timothy M. Dean (aka Tim Roy Williams), was
reportedly at the apartment of another man when he became

unresponsive. Emergency medical selvices were summoned, and Mr.


Dean's death was pronounced at the scene.

No overtly fatal trauma was identi~ied during the autopsy. The


heart demonstrated mild to severe ~oronary artery atherosclerosis
with up to 30-60% stenosis of the left anterior descending coronary
artery, up to 10-20% stenosis of the left circumflex coronary
artery, and up to 70-95% stenosis of the right coronary artery. An
anterior neck dissection was unrema.rkable. Pulmonary and cerebral
edema was present.

Microscopic examination of all major organs was performed. The


heart demonstrated mild inflammation composed of lymphocytes; no
definitive myofiber destruction was seen. The lungs demonstrated
edema and mild chronic inflammation involving some of the
bronchioles. The liver demonstrated chronic hepatitis, and the
kidneys demonstrated nephrosclerosis.

Microscopic examination of tissue from the right antecubital fossa


demonstrated a vein with adjacent hemorrhage, and the hemorrhage
contained a piece of colorless, translucent spherical material that
had the appearance of a Maltese cross when examined with polarized
light. Cornstarch, which is sometimes used to dilute drugs, appears
colorless on H&E stain and appears similar to a Maltese cross when
polarized. The tissue also demonstrated occasional multinucleated
giant cells that contained foreign material which was refractile
when viewed with polarized light, compatible with previous
injection of a substance containing insoluble particles.

Molecular testing of a nasopharyngeal swab did not detect any of


the more common respiratory viruses.

Toxicology testing demonstrated the presence of ethanol (0.128


gram% femoral blood), methamphetamine, and a low concentration of
amphetamine (a metabolite of methamphetamine) .

Coronary artery atherosclerosis results in reduced blood supply to


the heart muscle, which can result in an arrhythmia (abnormal
cardiac electrical conduction and contraction) or a myocardial
infarction (death of heart muscle due to insufficient blood flow).
Methamphetamine is a stimulant drug, and it can increase the heart
rate and blood pressure, thus increasing the work done, and oxygen

needed, by the heart. This can restllt in myocardial ischemia due to


the oxygenated blood need outstrip~ing the supply, especially in
the case of a preexisting lesion w1ich would limit blood flow, such
as the significant atherosclerosis involving Mr. Dean's coronary
arteries.

Additional information from the investigation of the death included


that other men who had reportedly previously been in the same
location where Mr. Dean was found had reported drug use via penile
injection. In this case, dissection of penile tissue did not reveal
evidence of injection or trauma; edema was present, but this was
compatible with the flexible plastic devices seen on the genitalia
in the scene photos.

The presence of an old injection site with new hemorrhage suggests


that this was at least one of the routes of methamphetamine use. It
was posed a possibility that someone else had injected Mr. Dean
with drugs. There were no signs of physical restraint or
incapacitation. If Mr. Dean was incapacitated to a degree by the
ethanol, it seems less probable that another person would inject
drugs into the exact same spot as an old injection site when the
drug use reported by other persons previously at this location was
through a completely different anatomic region.

The cause of death is methamphetamine and ethanol toxicity, with


coronary artery atherosclerosis as a contributing condition. Based
on the history, circumstances, and/or autopsy, as I currently know
them, the manner of death is accident (due to methamphetamine use) .

Microscopic description:
Cardiovascular System: The epicardium demonstrates patchy, mild inflammation composed of lymphocytes. The myocardium
demonstrates scattered regions of enlarged cardiac myocytes with enlarged nuclei. The myocardium also demonstrates the
presence of scattered lymphocytes with an occasional small aggregate of lymphocytes in a perivascular pattern; no overt
cardiac myocyte destruction is identified. Minimal wispy fibrosis is i4entified, There are no regions of overt hemorrhage or
necrosis. Sections of coronary artery demonstrate significant intimal thickening and fibrosis, mild inflammation composed of
lymphocytes is also present.

Respiratory System: Some of the alveoli demonstrate the presence of light pink lightly flocculent, to focally confluent,
material. Scattered regions of alveoli also contain macrophages. Some of the bronchioles demonstrate mild inflammation
which appears to be composed predominantly of lymphocytes with an occasional possible eosinophil (identification limited by
autolysis). No acute inflammation or hyaline membranes are seen within the alveoli.

Hepatobiliary System: The liver demonstrates an unremarkable architecture. Hepatocyte plates are 1-2 cells thick. Some of the
portal tracts demonstrate mild to moderate inflammation composed oflymphocytes; minimal, patchy interface hepatitis is
present. No necrosis or hemorrhage is identified. Steatosis is not identified. The gallbladder demonstrates autolysis but is
otherwise unremarkable.

Pancreas: The pancreas demonstrates advanced autolysis which precludes further interpretation.

Genitourinary System: The kidneys demonstrate mild to moderate autolysis; the kidneys also demonstrate mild interstitial
fibrosis and scattered globally sclerotic glomeruli. No inflammation !is identified, but the exam is limited by autolysis. No
significant crystals are identified with polarized light examination of the kidneys.

Endocrine System: The thyroid demonstrates mild autolysis as well as relatively well developed follicles which contain
colloid. The adrenal glands demonstrate congestion but appear otherwise unremarkable.

Hemolymphatic System: The spleen demonstrates fairly advanced autolysis; the architecture appears overall unremarkable,
but the exam is significantly limited by autolysis.

Soft Tissue: A section of subcutaneous tissue from the right antecubfral fossa demonstrates a vein with adjacent hemorrhage.
A few multinucleated giant cells are present in the vicinity of the veih, and polarized light demonstrates the presence of
refractile material within some of the giant cells. In a portion of the hemorrhage is a piece of translucent spherical material that
has an appearance of a Maltese cross when examined with polarized light. No significant inflammation is seen in association
with the hemorrhage.

brown and red residue as well as possible white, powder-like residue ("I")

Tite decedent was invited to a 1nan's apartment where he is reported to have begun exhibiting bazaar behavior. at one point using clothing to tie a noose around his
neck. The witness rep:irts that he re1noved the "noose" prior to leaving the room to take a shower. He found the decedent unresponsive when he got out and

On-Scene Investigation.
Investigation:
On 01/07/19 at 0215 hours Sgt. Cardella reported !~is case to our office. The scene was ready
for my response and I was assigned this field call a!I0725 hours. I arrived on scene at 0815
hours and concluded my scene investigation at 090(!) hours. Forensic Attendant Martinez
arranged for the transportation of the body to the Fo[ensic Science Center.
A fingerprint submission through the Livescan system returned a record with the United States
Department of Justice, Federal Bureau of lnvestigatibns indicating an arrest history for DUI with
alcohol.
Location:
Injury and death occurred at 1234 N. Laurel Ave #1' in West Hollywood.
Informant/Witness Statements:
Sgt. Cardella reports that on 01/07/19 the decedent was a guest in a man's apartment when he
began to exhibit bizarre behavior. The witness obsefved him using a piece of clothing to make a
noose and tied it around his neck. The witness relay~ that he removed the "noose" and the
decedent then stood up and began throwing clothes up in the air. At this point he relays that he
went to take a shower and when he returned he foun.d the decedent unresponsive. He relays that
he attempted CPR for 15 minutes before contacting 911. Los Angeles County Fire responded
and Paramedics determined death at 0136 hours. The witness reported that he did not see the
decedent taking any drugs and that they did not have sex.
Scene Description:
The scene is a second floor apartment. The body is seen in the living room lying partially on a
mattress that was on the floor in front of a sofa. The 'room is strewn with clothing and sex toys.
Three large mirrors were standing upright leaning against the walls. Plastic baskets were lined
up on the floor containing folded men's underwear. ~lso on the floor I observed two short pieces
of thick clear plastic tubing (each approx. 2 feet long), one appeared to contain a white powdery
substance. The refrigerator contained a syringe containing a clear liquid, 2 clear bottles with the
labels panted over with blue paint -found to be labeled Naloxone, one bottle labeled Caverject,
and a box labeled Alprostadil/Papaverine with a bottle containing a clear liquid. A glass pipe was
found on the dining room table and another was on the stovetop. Two more empty syringes were
found on a desk in another room. Note: I observed no items of clothing that appeared to be
fashioned like a noose. ·

Evidence:
Medical evidence was collected and booked into evidence. See form 3A for details. Other
evidence including a sexual assault kit was collected by Criminalist Chavez.

Body Examination:
The body is seen supine on a carpeted floor with the l.egs resting on the mattress. It was that of
an adult male with medical appliances present and ntj obvious signs of trauma. A dark purge had
emitted from the nose and mouth and was dried on th~ face. The body was in full rigor mortis
and wearing 2 pairs of white men's briefs - one of the~waistbands was pulled under the penis,
and 1 red jock strap that was askew. A wide silicone circular sheath was wrapped around the

base of the penis and rubber ring was wrapped arou~dthe scrotum. He had short black hair,
brown eyes with !ache noire, a mustache, and his own teeth. Tattoos were observed on the arms,
abdomen, and left lower leg.
Identification:
The body was identified after a submission of fingerprints through the Livescan system.

Autopsy Notification:
Sgt. Cardella requests notification prior to autopsy. See case notes for confidential contact
