In line with DepEd Order No. 22, s. 2013 entitled ‘Revised Guidelines on the Transfer of Teachers from One Station to Another” and to monitor and ensure the equity of teacher deployment, please be informed that the standard work flow procedure of request for transfer within the district or division shall be followed by all Elementary Schools, Junior High Schools and Senior High Schools 2. Transfer of teacher from main school to annex school or vice versa is not covered by this procedure. 3. Attached is (Enclosure No. 1) for the work flow procedure of request for transfer. Lucian, Trece Maries Cty Cavie [Text Ht Planning Officer Details © Prepares request letter for transfer based on DepEd Order no. 22, s. 2013 and forwards to School Head or PSDS * Prepares endorsement letter for transfer indicating the following information: a. Name of employee to be transferred b. Plantila item number of employee c. Present station d. Target station @. Reasonis of transfer (Note: Endorsement letter shall be signed by Public School District Supervisor for Elementary Teacher and School Head for Secondary Teacher. Forwards request for transfer documents [see attached checklist] to DHRMO for checking and then forward to Division Office for approval) Receives request for transfer from present schoolidistrict and prepares approval/disapproval form of request for transfer and forwards to Planning Officer for review and recommendation. «Reviews request letter for transfer and verifies if the transfer conforms to the requirement on computation of teacher:pupil ratio. Reviews endorsement for transfer if can be accommodated Prepares endorsement letter and forwards to Division Office. (Note: School Head/PSDS shall state the reason/s for disapproval of request on the endorsement letter if the requesting employee cannot be accommodated, 4g. unavailability of item and/or area of specialization is not needed in the junior high or senior high school) Receives endorsement from target school/district and endorse documents to present schooldistrict of employee. For transfer within the district, deploys requesting employee to target station if the request is approved and can be accommodated; if not, informs the teacher for the disapproved request. For transfer within the division, informs requesting employee to prepare and ‘submit requirements for Special Order of Transfer if the request is approved and can be accommodated; if not, informs the teacher for the disapproved request.
