AFFIDAVIT OF CONSENT

I, ADELFA B. DANILA, of legal age, Filipino citizen, and a resident of Brgy.

Cuambog, Mabini Compostela Philippines, after having been duly sworn in accordance with law,
hereby depose and state:

1. That I am the biological mother of MARY ROSE BAJAO, 16 years old

and was born on February 1, 2001;

2. That my sister ALONA B. KOBAYASHI is now in JAPAN and instructed

me to let Mary Rose Bajao to travel to Japan together with ALMER E.
BAJAO for a vacation;

3. That I have fully given my consent to the aforesaid passport, visa

application and all other legal requirements in order for her to legally travel
abroad;

4. I am executing this affidavit to attest to the truth of the foregoing narration

of facts and for whatever legal purpose this may serve.
