I am the registered owner/proprietor of the aforementioned business duly established and
registered under Philippine laws. I am formally signifying my intention to cease/terminate the operation of my business because it is no longer in operation since March, 2020, when President Rodrigo Duterte placed the entire country under Enhanced Community Quarantine (ECQ) due to the COVID-19 pandemic. Even though we shifted from ECQ to MGCQ, I preferred not to open my business because I feel that it is not safe yet and I don’t have enough resources to comply with the government’s standards in implementing the health protocols in order to prevent the spread of the virus in my establishment. I incurred huge loss because of the non-operation of my business and permanently closing the same is my only option.
