1. Licensed physical therapist providing intensive therapy at inpatient rehabilitation
facility for a variety of neurological, musculoskeletal, cardiopulmonary, orthopedic, and
multi-trauma cases.

a. Improvement in the aquatic usage: I assessed the issue of decrease utilization of
the aquatic therapy pool and determined that a significant cause was decrease
referrals from primary therapists. To address this, I led an in-service educating
the therapy staff on the benefits, appropriate usage, and process of pool therapy.
As a result, the referral rate increased about three-fold with a subsequent increase
usage of the pool.
b. Wheelchair cushion: The hospital determine that wheelchair cushions were a
needed capital expense. I led the project to make recommendations, which
included a hospital-wide wheelchair cushion survey to determine what cushions
were being used and where we were lacking. Once determined, I surveyed the
therapy staff about what they felt was needed in a cushion. I looked at different
costs and suppliers of the cushions and then complied the results and presented
the findings with my recommendations on the best cushions and costs for the
hospital
7. Engaging and educating the community in public health activities, promoting a life-long
wellness mindset
a. Teaching a class through the Oscher Life-Long Learning Institute on the benefits
of aquatic exercise for the elderly
b. Participating in various community health fairs with information pamphlets for
classes offered at Encompass Health
c. Leading aquatic exercise classes to community members
d. Supervised a Parkinson’s focused boxing class for members of the public
e. Teaching a lunch-in-learn to my co-workers as part of a PT month activity on
how to develop and sustain a good exercise program for life-long wellness

the primary contacts of each patients to keep them informed and address any
concerns they may have

them on safe moving and handling techniques and discharge recommendations
g. Writing letters of medical necessity for specialized adaptive equipment,
particularly high-end electric wheelchairs, that requires an in-depth knowledge
of insurance regulations in this area

looking at the latest scientific developments with this allied health profession
