Definition of

A. Journal Writing

Journal writing is the process of recording personal insights, reflections and questions on
assigned or personal topics. Journal projects assigned in class may include your thoughts about daily
experiences, reading assignments, current events or science experiments. Journal entries are a form of
reflective writing, in that you can use them to consider and respond to something you have read or
learned.

However, journal entries should not merely summarize what you have read, nor should they
focus only on your feelings. Instead, they should demonstrate your ability to conduct a critical inquiry.
The term, critical inquiry, refers to the steps involved in collecting and analyzing ideas or information.

B. Intake Sheet

An Intake Sheet uses the data collected to populate the Summary section (or profile data) of
your project sheets automatically.

The Intake Sheet standardizes and restricts access to profile data for each project. Locking items
like Budget or Project Due Date prevents team members from changing important data points.

The intake process is often the first encounter that a client may have with a social worker, so it’s
important to make a positive impression. Make the process easier for both staff and new clients by

C. Process Recording

Process recording is a tool used by the student, the field instructor, and the faculty advisor to
examine the dynamics of a particular interaction in time. The process recording is an excellent teaching
device for learning and refining interviewing and intervention skills. The process recording helps the
student conceptualize and organize ongoing activities with client systems, to clarify the purpose of the
interview or intervention, to improve written expression, to identify strengths and weaknesses, and to
improve self-awareness (Urbanowski & Dwyer, 1988). The process recording is also a useful tool in
helping to explore the interplay of values operating between the student and the client system, through
an analysis of the filtering process used in recording the session.

D. Transfer Summary

Transfer Summary means the written document which precedes or accompanies an patient-
inmate upon a patient-inmate’s discharge from a hospital to a skilled nursing or intermediate care
facility, Correctional Treatment Center, or to the distinct skilled nursing or intermediate care service unit
of the hospital where continuing care will be provided. The transfer summary, signed by the attending
physician, includes the following information relative to the patient- inmate’s 1) diagnosis 2) hospital
course 3) medications 4) treatments 5) dietary requirements6) rehabilitation potential 7) known
allergies 8) treatment plan.

E. Project Proposal

A project proposal is a document that is used to convince a sponsor that a project needs to be
kicked-off to solve a particular problem or opportunity. It describes in depth, how the project is going to
be commenced so that the sponsor understands what is involved early. Whenever you start a new
project then you need to complete a project proposal document. The document is provided to the
sponsor of the project for approval. Only then can the project officially commence. A project proposal
may also be used to set the roadmap for the project, so that everyone is working towards the same
goals.

F. Social Case Study Report

Social Case Study Reports are required by charitable institutions, government hospitals and non-
government organization that provide services to clients and patients. CLIENT GROUPS: Clients/patients
(Immediate family of the patient).

Case studies are an in-depth investigation of one particular individual, group, time-period or
event. They encompass a range of qualitative and quantitative research tools to investigate underlying
principles of an occurrence within a real-life context.

G. Work and Financial Plan

Financial planning is focused on helping individuals meet life goals by managing their financial
resources using various strategies and tools that fit their circumstances. Financial planning is
characterized as a largely upper middle-class endeavor focused on goal realization, whereas lower-
income individuals have needs related to cash and debt management.

H. Transfer Summary

I. Closing Summary

The Closure Record summarizes the impact of actions and services provided to a child and
family, and the evidence that supports the decision to close a case. This is used when it is planned to
cease work with a child or young person and family.

J. Case Management Process

The Case Management Process is centered on clients/support systems. It is holistic in its


handling of clients’ situations (e.g., addressing medical, physical, emotional, financial, psychosocial,
behavioral, and other needs), as well as that of their support systems. It is adaptive to case managers’
practice settings and the settings where clients receive health and human services.

The Case Management Process consists of nine phases:
