Sub: Work order for printing & supply of 4,000 MICR Dividend Warrants for the year 2019.

With reference to your quotation no. hisofl/Quot/200809-01 dated 09.08.2020, we would inform you that
the Management of the Bank is pleased to accept your offer for printing of 4,000 (Four thousand) MICR
Dividend Warrants as per following terms & conditions:-

As such, you are requested to print 4,000 (Four thousand) MICR Dividend Warrants with text
personalization as per following manner:
1. 2,422 numbers of MICR dividend warrants to be printed starting serial no. from 0000001 to
0002422 containing with required data print.
2. Rest 1578 numbers of MICR dividend warrants to be printed without data print (i.e. name of
shareholder, FoliolB.O. number, address, amount, date, name, designation & signature of
Company Secretary etc. should not be printed).

Delivery of Dividend Warrants must be completed within 3 (three) days from the final approval of the
sample (machine print).

Yat & Income Tax will be deducted from the bill amount as per Govt. rules.
