AFFIDAVIT
I, JOY A. EBRADA filipino, of legal age, single and with residence and
postal address at Barangay Poblacion Norte, Municipality of Barcelona,
Province of Sorsogon under oath depose and state:

1) That on January 7, 2020 at around 6:49 in the morning I gave birth to a

baby boy while I was riding a transport vehicle from our house at
Barangay Poblacion Norte, Municipality of Barcelona going to Sorsogon
Provincial Hospital;

2) That in going to the hospital, I was assisted by the father of my child,

Angel Joseph Labasano, and Ms. Josephine Erlano a licensed midwife
from Gubat, Sorsogon;

3) That after delivery, the father of my child and I decided not to proceed to
Sorsogon Provincial Hospital instead stayed at my house since I was
already attended by Ms. Josephine Erlano a licensed midwife;

4) That I am executing this affidavit as a necessity in processing the birth

certificate of my child;

5) That I am attesting to the truth of the foregoing instrument and for

whatever legal purpose this may serve;
