Catabayungan, Cabagan, Isabela, after having been duly sworn to on oath in accordance with
law, do hereby depose and say:

That we personally know one PAZ ONG-RAMOS or AEROPAGITA ONG-RAMOS,

AREOPAGITA ONG-RAMOS, EUROPAQUITA ONG-RAMOS and ORAPAGUITA
ONG-RAMOS, the legitimate spouse of FELIX RAMOS, also of Catabayungan, Cabagan,
Isabela;

That we have personal knowledge of the fact that PAZ is also commonly called by her
relatives, friends and acquaintances as AEROPAGITA, AREOPAGITA, EUROPAQUITA
and ORAPAGUITA;

That in our community in Cabagan, Isabela, PAZ ONG-RAMOS is also known as

AEROPAGITA ONG-RAMOS, AREOPAGITA ONG-RAMOS, EUROPAQUITA ONG-
RAMOS and ORAPAGUITA ONG-RAMOS;

That in our barangay in Catabayungan, Cabagan, Isabela, there is only one person bearing
or is being known by the name PAZ ONG-RAMOS or AEROPAGITA ONG-RAMOS,
AREOPAGITA ONG-RAMOS, EUROPAQUITA ONG-RAMOS and ORAPAGUITA
ONG-RAMOS;

That our personal knowledge stems from the fact that PAZ ONG-RAMOS or
AEROPAGITA ONG-RAMOS, AREOPAGITA ONG-RAMOS, EUROPAQUITA ONG-
RAMOS and ORAPAGUITA ONG-RAMOS and her parents have been our long-time friends
and acquaintances and we move and live within the same circle in he community;

That we are executing this affidavit in order to attest to the truthfulness of all the
foregoing statements.

spouses ALFONSO ONG and CONCEPCION MALLANAO;

4. That she was married to FELIX ALLAM RAMOS with whom they were blessed with

2. That we personally know spouses Virginia Totto Baquiran and Pepito Agabin
Baquiran who got married on June 6, 1978 at Sto Tomas Isabela;

3. That the relationship of Pepito Agabin Baquiran and Angelina Gamboa has been
living together as husband and wife without benefit of legal marriage and they have
been blessed with one child namely Patrisha Mae Gamboa Baquiran and presently
residing at Sitio Cracer, Sapang II Ternate, Cavite;

4. That we know the above-said fact because we are relatives;

I, REBECCA GRACIOSO VETONIO, of legal age, Filipino and resident of San

Bernardo, Cabagan, Isabela after having been duly sworn to in accordance with law hereby
depose and say That:

1. I am the same person executing this affidavit;

2. I was born on January 10, 1950 at Allen Northern Samar;

3. My certificate of live birth was not registered with the Local Civil Registrar Office of
Allen Northern Samar due to negligence;

4. That this affidavit is being executed to attest to the truthfulness of the facts
surrounding my birth and to support my application for delayed registration of my
birth pursuant to Rule 20 of Administrative Order No. 1, series of 1993 which is the
Out of town Reporting of birth.
